I read your query and I understand your concern.
That MRI report doesn't show any acute lesions, nothing threatening in short term. However it shows the remains of a small
stroke, which you have had a long time ago. Perhaps you already knew that, but if you didn't it is not surprising because it is a small stroke and could have happened without major noticeable symptoms. There are also small scattered changes related to narrowing and occlusion of small vessels.
This report indicates that you are at risk of having a stroke in the future so you should pay attention to risk factors for that and address them. Risk factors include
high blood pressure,
diabetes,
smoking,
high cholesterol, obesity, sedentary lifestyle. Correcting them will lower the risk for stroke in the future.
It would be useful to know your age. These findings are frequent in the older population, but if you are young and without risk factors known then you should also be evaluated and tested for other rarer causes like inflammatory conditions, metabolic conditions, genetic inherited conditions etc.
