Hi, Dr. female age 40yrs . I have symptoms like weakness, under Wight I have done thyroid blood test. THYRIOD PROFILE SERUM (CLIA) T3, TOTAL SERUM: 0.10 ng/ml T4, TOTAL SERUM: 0.50 ug/Dl TSH SERUM: 1.61 ulU/ml i need your advice. thanks
Your hormonal profile shows that you have central hypothyroidism, ie low t3,t4 and inappropriately normal TSH. So the defect lies here in the pituitary. But isolated TSH deficiency is a rare entity. So you should make sure you don't have a hypocortisolism for which you have to undergo a short snatched test. And also you did not mention about your menstrual cycles, whether you stopped having it. If you have low cortisol, then treat it first before managing hypothyroidism as it can lead to adrenal insufficiency otherwise. In such cases,eye examination and pituitary imaging is worth doing.
