40 yr old male with complains of weakness in lower limbs since 1 1/2 month,history of 1-2 falls with minor limb injuries, delayed response to verbal interaction since 1 month and urinary incontinence since 3 days.

Weakness of lower limbs with urinary incontinence means there is a problem in the spinal cord at the thoracic ( Mid back ) level. It could be either obstruction to the blood flow to spinal cord , infection of the spinal cord , some mass or tumor compressing the spinal cord .
You need to consult a Neurologist for this problem . He shall advise for a MRI thoraco-Lumbar spine to see for any structural problem in spinal cord , he may also advise for a test called CSF analysis which will look into infection of spinal cord .
