Infertility due to failure to form eggs (ovulate) is one aspect of PCOS. Hyperinsulin state and Hyperandrogen state are the other two aspects of it. While treating PCOS, one has to have a comprehensive management of these.
You have mentioned your weight but not the height. Considering your weight, it is likely that your BMI may be rather high. Please calculate your BMI. While undergoing treatment for PCOS, you should try to keep your BMI between 18 and 23.9. High BMI is known to cause failure of PCOS treatment in achieving pregnancy. High protein, moderate
carbohydrate and low fat diet along with exercise is the key to control BMI. Metformin also helps bringing down BMI.
For induction of ovulation, there are various medicines which are being used. Initially we try Clomiphene citrate for about six months. If it does not work, the
Gonadotropin hormones may be tried. Since your periods are regular, there are chances that you may be ovulating also. Hence, it seems that your doctor has not added any medicine for
ovulation induction.
You have been prescribed Glyciphage which is Metformin. This medicine is for controlling the hyperinsulin state by increasing the sensitivity of the tissues to the insulin.
Folic acid is a good support to have good quality egg and also to ensure good growth of the baby in early stages of pregnancy.
Along with this treatment, I would advise you to keep on monitoring your ovulation by
ultrasonography. You will have to get scans done from day 9 onwards till you ovulate. This will give you an evidence of ovulation and accordingly you may plan your sexual intercourse. Along with, scan will inform you the size of follicle when it released egg and also the thickness of endometrium on the day of ovulation. Follicles of size between 18 mm and 21 mm release the healthiest eggs who have best chances of taking the pregnancy to success. Also, endometrium between 9 mm to 14 mm size is most conducive for
implantation of the fertilized egg (blastocyst).
If you are not ovulating, you may have to go for Clomiphene citrate and Glyciphage together.
