As you have been on treatment for
psoriasis for 8 years, do keep in mind the factors which usually cause it to flare up. It maybe medicines, weather changes, certain foods, alcohol,
smoking. etc. certain antibiotics can cause a flare up.
If
protopic is helping you and not increasing your rashes do continue it. Don't stop any treatment abruptly. As for penicillin, do always consult your doctor before stopping as it is required to be taken for a long period of time in regular doses and has shown good improvement. Any treatment stopped before the lesions clear up completely can cause it to flare up again.
Since your disease is only of the ankles it is easier to treat. As you are developing a new rash, the first consideration would be psoriasis. try and keep a note of anything that could have caused a flare up. Start applying protopic on the lesion. Check for improvement.
You might need antihistamines if you have itching. Also a short term of
azithromycin or
roxithromycin can be considered for relief.
But if it persists or looks different from the psoriasis, do get it looked at. It may be an
allergy, infection etc.
In any case use the treatment prescribed regularly for a long time till it all clears up and then slowly stop. Keep a check on your diet an lifestyle. Avoid factors that trigger your disease.
Also consider treatment such as UV
light therapy etc.
