The history of pain in abdomen with pressure on stomach on using washroom, frequency of urination, back pains , sharp pains in the abdomen and body pains is suggestive of a severe
urinary tract infection along with possible secondary changes.
Urine is showing blood in the urine indicating the severity of the infection and chance of severe
cystitis or
pyelonephritis.
The fainting and weakness are secondary to the infection.
I would advise you the following:
First of all send the sample of urine for culture and sensitivity before the antibiotic is started to get an appropriate results.
Get all the relevant tests of blood for WBC, urea,
creatinine, sugar and so on.
Ultrasonography is the single most important investigations to tell us the changes in the urinary tract so that further treatment can be planned. Intravenous pyelography may be needed as is the cystourethroscopy.
Get started with proper antibiotic for 3 weeks / till get alright.
Urinary alkalizer with water thrice daily.
Increase the intake of oral fluids till you can tolerate.,
Further treatment as per the findings on clinical and lab evidence.
If surgical cause is found , get operated.
