What Should Be Done For My Elderly Father With CKD, Hypertension, Abnormal Creatinine And EGFR?

My father is 68 years and he was detected with CKD in 2019. He has a history of hypertension and is taking BP medicines. Didn t consult doctor since 8-9 months. Yesterday his tests were done. His current creatinine level is 1.58 and eGFR is 47. Kindly suggest the course of action.
