This medication is used to treat vaginal fungal infections. I presume your midwife has performed a local examination on the vagina before prescribing this medication. If not, please see a specialist for proper management. Also, bleeding on passing stool is related to the anal canal and not the vagina. Sometimes, local causes like erosions, polyps and infections also cause spotting. A fissure in the anal rim, hemorrhoids-both internal and external, also can cause such spotting and bleeding.
