enlarged cardiac silouette- means that the heart is bigger than it is supposed to be.
vascular congestion of lungs means that there is some sort of back pressure from the left heart to the lungs, due to which blood in the lung vessels has increased.
These two features is suggestive of a congenital heart disease in a 20month old baby.

enlarged cardiac shadoq with congestion is due to a block or an impairement to the normal flow in the heart. It could be due to a congenital heart condition which can be diagnosed by proper investigation. I would advise you to get a 2d echo of the heart and visit a cardiologist in this regard.
