In left ventricular failure, heart is unable to pump blood adequately from the left ventricle. which lead to pulmonary congestion i.e leads to accumulation of fluid into the terminal respiratory unit. It leads to dyspnea. It is our body reeflex mechanism that it want to clear the secretion from the lungs thats why person is having cough.

The answer by our colleague is perfect ;i just want to add that this cough due to LVF is increased on even slight physical exertion;like walking ;straining for stools.The patient is better in propped up position & has difficulty when lying down .This cough is not reduced by any medicine other than Diuretics.Once the Diuretics help to remove excess of water in lungs the patient feels better.I just elaborated my friends answer as you are MBBS student & in future benefit academically.
