Why Does One Experience Shortness Of Breath And Pulsating Sensation In Chest?

Every now and then i get a thump in central chest just below collar bone and shortness of breath .the first time i noticed it was when i went to spain in oct this year.i am over weight and asmoker 20 awk .i do exercise alot but only sleep for a solid 3hrs rest of my sleep is broken
