Why Does One Reach Climax Quickly During Sexual Intercourse?

I find that i reach climax point really quick during masterbation ,say less than a minute,not sure if its bcause i masterbate fast but also during intercourse,after foreplay when my penis is perfectly erect,when i insert maximum 3 or 4 thrusts i give and climax is reached. Not sure if its because of the rough skin around my penis head..maybe the friction is too much.?

Thank you for writing to HCM.
I understand your concern for rapid climax (premature ejaculation).
Premature ejaculation occurs commonly during early sexual encounters.
Sometime due to fatigue, lack of mood, situational problem or environmental changes man can fail in sex. It is normal phenomenon. This is called performance anxiety. After such failure when person actively thinks about erection or success of intercourse during sex/masturbation his mind is likely diverted to such thoughts.
For constant erection continuous exciting/romantic/erotic feelings should be there in mind. If mind diverted to concern about erection/ejaculation, ejaculation occurs fast.
When clients with premature ejaculation come to my clinic, I recommend them one or more of the following strategies which often help them.
1. Stop-start technique of behavior therapy.
2. Female on top with active role.
3. Double condom
4. Medicines like Fluoxetine 20 mg day or Paroxetine CR 25 mg per day.
(To be taken under medical supervision only)
5. Kegel exercise also helps in some cases. You can learn the same from internet. (As if try to hold urine intermittently during voiding)
6. Penile squeeze technique applied at the point of ejaculatory inevitability.
7. Applying a local anesthetic ointment to glans penis prior to penetration.
8. Leading partner to orgasm by exploring and stimulating other erogenous zones. (Other than vaginal containment movements only)
9. You should do masturbation before sex (at least 2 hours before). It will make seminal vesicles somewhat empty. So, at the time of sex, semen will take more time to come out.
One can gain ejaculatory control over a period of time by practicing behavior methods mentioned above REGULARLY. Practice makes a man perfect.

Understand your concern
you have premature ejaculation that is mild form.
Following are the causes: stress, sexual anxiety, smoking and alcohol consumption, fatigue, diabetes and hypertension.
You have to consult the urologist for proper evaluation.
Take following advice:
Be relax while doing sex, do masturbation before sex, use double condoms or condom with lidocaine jelly that decrease penile sensitivity, hold urine before urination, do pelvic exercise, try women on top position, use start and stop technique of sex (divert the mind from sex at the time of ejaculation) and squeeze technique.
If you have stress then take cognitive behavioral therapy for cognitive dysfunction, start regular exercise like walking and medication 30-50 minutes daily in the morning, take healthy diet rich in protein, take antioxidant rich almond, fresh fruits and Co Q, stop alcohol and smoking, tablet gonadil F. If above advice will not work, then take sildenafil plus paroxentine 30 minutes before sex after proper evaluation.
