Dr. Raghu, You have been a tremendous help. It's hard to believe that the specialists I have seen could not determine this. I like to think they did but chose not to tell me. Of course I am not medically educated at all, but it sounds like this should be Orthopaedics 101. I plan to do exactly as you recommend.
