Hi XXXXXX
Thanks for writing in.
1. The cause of the abdominal pain is what is termed as infant colic. This is a known happening in most kids. The reason for this is that the intestinal flora or organisms start growing in numbers. These are beneficial for the absorption of various vitamins and nutrients. As your son has just recovered from an episode of intestinal infection it is expected that the pattern of growth of these organisms is altered or skewed. It will take some time for this to resolve. I would suggest that you withold weaning in case you have started it. Because with only breastfeeding he should have the least of problems. Dimol is medication which relieves flatulence. Abnormal action of organisms causes more gas and thereby distension of the intestine giving rise to sudden episodes of colic. Dimol is not a toxic drug in any way. It is quite safe to use it even for a longer period. As the flora develops the incidence of pain episodes should decrease and the need for dimol will not arise.
2. Sleep in kids is related to spurts of growth hormone release in the body.It is a natural process. You do not have to worry as long as the child's apetite is good and the child is playful when awake. If these 2 aspects are ok then there is no need for any intervention in this respect. Sedatives are harmful for the development of the childs brain.
I hope this answers your querry.
If you have any further doubt please do write to me. I can understand the stress a mother has to go through.

Dear Doctor,

Thanks for your answer. Just couple of queries against your answer, as I'm not clear.

For your 1st answer:
1. When you are syaing that due to intestinal infection pattern of growth of these organisms is altered or skewed - Will it be a problem to growth my son?

2. My understanding for the reason of Colic is child's poor apetite. Is it correct?

3. If I undersatnd correctly, you mean to say I'd need to continue with breast feeding & If we continue use the Dimol, there is no side effect for it?

For your 2nd answer:

1. I understand, Sedatives are harmful for the development of the childs brain. But good thing is that my son is playful when awake. But I've doubt whether he has very good apetite. Is there any other way to improve his apetite power?

Pls note - following 4 medicine I'm currnetly applying

1. Zincovit syrup
2. Neopeptine drops
3. Bevon drop (It would be starting from today, as advised by physian)
4. Dimol Drops (when required)

Is there any thing else that I can use for my baby to improve his apetite?

Hi,
Thanks for the feedback.
1. This is a temporary phase. Once the infection subsides within due course the normal indigenous helpful flora grows and symptoms disappear. But be careful that the kid does not develop gut infections frequently. Frequent infections can cause problems by way of malabsorption. Otherwise it will not hinder growth at all.
2. The mechanism of infant colic is the continuing process of development of the gut flora. In the normal course it would not affect apetite to a great extent. But if infection develops then the apetite is likely to suffer.
3. Yes, you should continue breast feeding. That will help in developing good gut immunity and has a protective effect by gaining readymade antibodies from the mother. In fact breast feeding will aid in better recovery and the need for prolonged use of dimol may not arise. Dimol as it stands is not a toxic medication.

Apetite can be improved by good vitamin supplements. The first medication which you have mentioned is good enough. You can try xymex drops in the dose of 0.5 cc once a day after the first feed.
I hope all your querries are answered.

Hi,
thanks for writing in.
The mild febrile reaction suggests a good response to the vaccine. I would suggests ice compresses to the site of infection and paracetomol syrup only if the kid is having fever. Yes it is safe for occasional use.

Hi,
Thanks for writing in.
1. White coating of the tongue if observed in the morning after getting up may suggest mild amount of indigestion. But if observed during day time in between the feeds is not of any medical consequence. As long as the kid is feeding well and having good bowel habits and clearance you need not worry.
2. Regarding white spots on the inner side of the oral cavity I would suggest vitamin drops as already advised. I would also advise you as a nursing mother to take good quality of vitamins and food additives which will fortify the quality of beast milk.
For you I would suggest the following
1. Cap Astymin forte 1 cap after breakfast.
2. Cap cobadex forte 1 cap after breakfast
3. cap mega 3 1 cap after breakfast
4. T calcium lactate 1 tab at bed time
5. cap softeron 1 cap after breakfast.
Plenty of fluids and a high protein diet.

This you can continue till you continue to breast feed your kid.

3. Do not apply any medicine to your kids mouth or tongue at all.
4. see the response to the above advise and only if the response is suboptimal you can have a pediatrician consult.
i hope this answers your querry.

Dear Doctor,

Thanks for the answer. However, can you please reply with the below queries on your answers

1) You have suggested to use vitamin drops (as part of Answer -2). Please note - I'm using Zincovit syrup to my son. Is it good enough to mitigate the white spots on the inner side of the oral cavity? If this vitamin is not good enough, request you to please let me know which vitamin can be used to my son?

2) As a nursing mother, which one I'd need to take to fortify the quality of beast milk - any one among the 5 you mentioned?

3) Pediatrician advised to apply ''KANSEL-MP Mouth Paint'' to my son. I'm afraid as you mentioned not to use any medicine to my son's mouth or tongue at all.

Please advise what should I do - will it be a problem if I use ''KANSEL-MP Mouth Paint'' to my son?

Hi,
thanks for the reply.
1. Zincovit is good enough but if you can get abdec vitamin drops that would be better. It will take some time for the white patches to disappear. Do not worry.
2. You need to take all the tabs which I have advised everyday. that will ensure the best nutritional fortification of breast milk.
3 Do not apply any oral medication for the time being. Only if the response is suboptimal then one can consider this application. Not to start with.
I hope this answers your querry.

Hi,
thanks for writing in.
In case if your kid is experiencing pain while feeding then it would be advisable to use the medication . But remember just use it once a day only at night. The agent works as a demulcent thus enhancing healing of the superficial raw areas.
i hope this answers your querry.

Hi,
thanks for writing in.
Demulcent effect is one in which there is precipitation of proteins which offer a thick coagulum like coating to the raw area thus helping in healing of the raw area.
Mouth paints act by three ways
1. Local anaesthetic effect thereby reducing pain.
2. demulcent effect which helps in healing
3. antiseptic effect which helps in preventing infection.

I hope this answers your question.

Hi,
thanks for writing in.
1. Cold and cough are self remitting viral infections. supportive care by giving symptomatic treatment and keeping the kid warm is very important. Antibiotics can be given but should not be more than 5 days in course duration.
2. Instead of taxim o a elementary antibiotic like kloxacillin would suffice. It would be preferable to administer these medicines orally.
3. Yes keep the kid as warm as possible. It would be preferable to sponge the kid rather than bathing the kid till all his infection disappears.
4. Vaccination is not advisable when the kid is having any infection. I would suggest that you wait till the infection settles down and the kid starts feeding well.
I hope this answers all your querries.
