Hello and welcome to Healthcare Magic. Thanks for your query.

Though masturbation is considered as a part of normal human sexual behaviour, it can become a problem when it becomes excessive and addictive. In such cases, it can result in excessive guilt and anxiety, and therefore affect the overall well being of the person. So, I would give you the following suggestions to overcome your problems of masturbation addiction and porn addiction:

- Firstly, try to keep your mind active and occupied at all times. When the mind is idle, it will tend to wander away to sexual thoughts. So, keep a busy scedule so that you don't let your mind have any idle time. You can find a new hobby or learn something new (like a new sport or a musical instrument) or you can take up a new assignment at work.

- Regular physical exercise is very important. You can join a gym or have your own exercise schedule. This will not only keep your body healthy but serve as a good outlet to let out to vent out your energy.

- Make specific plans and goals for your future. You have to make short-term and long-term goals for your life and start working to achieve them. This will keep your mind foccussed and prevent your thoughts from getting scattered.

- Improve your social life. XXXXXXX friends and relatives, go for outings or group activities and keep yourself busy. If you keep staying alone in your room, you are more prone to engage in masturbation frequently.

- You can try yoga and meditation. This can help you develop a discipline of your mind and body and help you get a mastery over your thoughts and behaviours.

- Remove anything which triggers you into a mood to masturbate. For example, clear your room of all pornographic material; if you access internet pornography, you can disconnect your internet connection, etc.

- Stop thinking and feeling guilty about this all the time. The more you think about it, the more difficult it becomes to resist. So, get rid of the guilt in your mind and keep your mind relaxed.

Regarding your other sexual problems of erectile dysfunction and premature ejaclation, there can be a variety of causes for these problem - both medical causes as well as psychological causes. So, the first and most important step in management is proper evaluation. This will include a detailed medical, sexual, personal and social history, a thorough physical examination and certain investigations, as per the necessity. This is both informative to the doctor and serves as an opportunity to educate the patient and clarify his doubts.

Firstly, any medical causes (like diabetes, hormonal problems, blood flow problems, vitamin deficiencies, etc.) need to be ruled out. For this, you may need to undergo a certain tests, according to necessity.

Having said this, I would also like to let you know that in more than 80% of cases of erectile dysfunction, psychological factors also play a major role in causing or aggravating the problem. Stress, anxiety, fears and doubts regarding sexual performance, feelings of sexual inadequacy, self-doubts, guilt, past negative sexual experiences, etc. can all affect sexual functioning and performance.

Treatment is targeted according to the findings on evaluation. If there are any underlying medical problems resulting in your sexual dysfunction, then those problems have to be first treated. If the problem is more psychological, then removing your anxiety, stress, doubts and fears, and improving your self-confidence levels will definitely help in improving sexual performance.

In my opinion, you seem to be having a lot of anxiety and stress, probably due to the guilt regarding excessive masturbation and its impact. Based on my clinical experience and expertise, let me assure you that if you control your unecessary anxiety, overcome your masturbation and porn addiction, and improve your self-confidence, then most of your other sexual problems will get solved automatically.

So, I would advise you to consult a psychiatrist or a sexologist in person to have a detailed evaluation and further treatment and counselling. There are effective psychological therapies like behaviour therapy, aversion therapy, hypnotherapy, etc. which can help you get control over your addictions. Also, avoid alcohol, smoking or any unnecessary medication and try to stay stress-free and positive.

Wish you all the best.

Hello again,

If your thoughts (regarding hurting others) have the following characteristics, then they are likely to be obsessive thoughts:
1) They are unwanted thoughts
2) They are repetitive and intrusive (that is, even if you don't want to think about them, they keep coming again and again)
3) You realize that such thoughts are irrational (that is, you are able to know very well that such thoughts don't make any sense)
4) You try to control or resist them, but without much success.

So, if your thoughts fit into these descriptions, then you are probably suffering from OCD and you need to see a psychiatrist. There are effective medication and psychological therapies which can help you overcome such obsessive thoughts.

However, if your thoughts are not obsessions and rather you have a voluntary feeling or desire to hurt others, then it is very worrying, and you need to get professional help urgently.

Wish you all the best.
