Human body has tendency to make phlegme normally and such sputum is expectorated outside.

Had he undergone open heart surgery got aortic valve replacement?
Presence of wound on chest wall may cause severe pain and difficulty in coughing phlegme.
You should consult with treating doctor and should go for chest x XXXXXX & ECG to rule out any XXXXXX organ injury.
If both of these investigations are normal then there is high chances of muscle pain and should take muscle relaxants with anti-inflammatory.
