Are Tri-Dot And Alere Trueline Tests At 6 Weeks Post Exposure Conclusive?

default
Posted on Mon, 10 Aug 2015
Question: Dear Doctor,
I would like to solicit your second opinion on my situation.
On 31 May 2015 i had protected sex. After returning i discovered an open wound on both my groin caused by rubbing against my underwear. I believe this wound happened in the morning and the sex was around 8pm. The wound is not that deep but it stings when I applied hand sanitizer to see if it is still open. I am scared the vaginal fluid might come in contact with my wound.
I had tridot test after 3, 4 and 6 weeks. On the 6th week, I had Tridot as well as Alere Trueline test together. All came out negative.
Since Tuesday I have a cough with yellow mucous in the morning. There is no blocking of the nose or throat pain. My friend who usually hangs out with me has the same symptoms and he's feeling well now. I havent had a temperature higher than 99.4F and now the highest is around 99F at around 4-5PM.
How high of a risk do i have of getting HIV?

Brief Answer:
No risk

Detailed Answer:
Thanks for your question. I am Dr Matin and will be answering your question.
Basics first : Symptoms have no meaning unless accompanied with a positive HIV report.
For successful transmission of HIV, the skin /mucous membrane need to be in a pronged contact with secretions of HIV infected person.
Taking these two basic facts , I can say that there is no risk of HIV in your case.
Hope this helps

Thank you.
I was concerned that the vaginal fluid could come in contact with the wound on my groin.

My other query is regarding Alere True Line test. This test was done on the 6th week. However I asked for Tridot and they did the test using that as well.
I tried to find information regarding Alere True Line but i did not find much information. The only ones i foundare Alere Determine and the Alere True Line i found was for Malaria Ag.
1.So is Alere True Line a 3rd Generation Test and does it test for antibody and antigen as well?
2. How accurate are the tests done after 6 weeks with Alere True Line and Tridot

Also, the 6th week test was done 46 days after the suspected exposure. Will there be HIV antigens that can still be detected at that time?

Detailed Answer:
Thanks for your questions.
1.Alere is a rapid point-of-care(POC) test that detects both HIV-1/2 antibodies and free HIV-1 p24 antigen. It 4th generation test which has the ability to identify HIV earlier than 2nd and 3rd generation antibody only tests.
2.The sensitivity of these tests @ 6 weeks will be 95%.
3.No , antigen will not be detected by the time line of 46 days.
Thanks
Hope this helps.

Thank you,
As you can imagine, i am quite stressed out.

1.I forgot to mention the cut was around 2 inches on both sides of my groin. I estimated it's around 12 hours between the cut and sex. Do you think it would have closed during that time? And with the additional information I provided. What are the odds of infection?
2. What is the earliest time I should go for retest?

Also. Can there be a time period where both antigens and antibodies are not available? (Antigens start becoming less after initial infection and antibodies are not yet created)

Brief Answer:
The contact has to be prolonged enough for transmission.

Detailed Answer:

1.The contact has to be prolonged enough for transmission. By 12 hours your wound would have started to close and unfortunately there are no studies to calculate your odds under your specified scenario, go for basics -you need a pronged contact with infected fluids and here the status of the partner is even unknown .
2.The protocol is the one test at base (which may be any time you decide to go for test after exposure) and another at 3 months of exposure to cover the window period of the virus.
3.No , antigens decline but antibodies stay through out the infection.
Hope this helps.

Thank you for your replies.

I want a conclusive test now. What test should I do?

Detailed Answer:
Thanks for your question.
The conclusiveness depends on the protocol and not on the tests as such.
Any test done as per protocol is conclusive which is - the first test at base (which may be any time you decide to go for test after exposure) and another at 3 months of exposure to cover the window period of the virus.
Hope this suffices.
