Are Muscle Twitching, Cold And Sore Throat Indicative Of Rabies Infection?

Question: Hi!! I got scratched by a dog in my great toe on April 11th. The dog was totally fine in the 10 days observation period. But from may 3 or 4th onwards d dog started showing signs and symptoms of rabies and den died on may 9th.the scratch was a minor one with no bleeding. So I din't take PEP. So after d dog died I got scared and started pep from may 10th and completed all 5 doses.but now from 2 days onwards I hav pain on my great toe ( scratch area) ,pain in calf muscle, muscle twitches on leg n biceps. Flu like fever from today wid cold, sore throat. Am I at any risk?

Hi, I am Dr Anshul Varshney , I have read your query in detail , I understand your concern.

The fever and the current symptoms that you have been describing seems more of a viral illness unrelated to rabies.

There is also a possibility that muscle twitches you are getting can be because of Anxiety.

Also, the dog died after 1 month of the exposure, that is a very rare event.
Moreover, the exposure that you had was very low risk as body fluids need to come in contact to cause transmission of rabies.

I would say, that you should see a Physician nearby you who would examine you and prescribe you for fever.
So, just relax. Don't get anxious. You would be fine.

I read somewhere if d dog is fine during 10 days period den it means it dsnt hav rabies. Is it tru? Is it possible for dogs to shed virus in its saliva before symptoms of rabies arises n if d dog was healthy at d time of exposure? If d exposure was severe (suppose dog bite) den shud I fear evn though the dog died after 28 days ? How long does the disease last in dogs after it develops symptoms of rabies?

Dear Friend, I understand your worry, but i would advice you not to worry.
yes, you have read it right that dog doesn't survive after 10 days, if it has rabies.
yes, a dog can shed virus in it's saliva before showing the symptoms, but it would die with in 10 days.
If the exposure is severe, we treat the person with Immunoglobulins along with anti rabies vaccine. So, that is not valid in your case.
Relax, see your doctor and get examined for fever.

So I shudnt fear as d dog showed symptoms after 24 days and died after 28 days?? So can it be possible that d dog apparently healthy was shedding virus in its saliva at the time of that exposure? The dog stays in our hostel with two other dogs. But the other dogs are healthy till now . The dog had hypersalivation , slept in a corner in the dark,dint come out ,and ran around aimlessly wen ders loud sound. So we suspected rabies. Could the dog hav died due to other condition? He's not an adult dog and neither a puppy.. a bit small than d adult dog. N the other dogs r healthy. I am scared and depressed. The incubation period is so long n variable that I hav to wait for 25 years to be certain. I am even having pain and a different kind of sensation in the area where I was scratched. Also I am having tingling sensation on my little and ring finger of left arm . Fever subsided a little. I completed my PEp. So taking PEP in the incubation period before virus enters CNS will help or is it too late? Also I took 5 doses of PEP each on 2007 and 2010. So is there a chance that d antibodies are going to help me this time? For how many years do the antibodies last? Also I want to know for a person who has been vaccinated previously why is that the person if reexposed takes only 2 doses? Why not 5? Is der any time limit ( within how many years of previous vaccination can he take only 2 doses and not all) ? Also previous vaccination only 5 necessary or booster needed for taking only 2 doses in d next exposure? Should I take d 90 days booster dose? Please help.. n please rpl to all my qstns.

Hi, The symptoms you have been mentioning in the dog seem to be of Rabies only. However, it is a possibility that the dog didn't have rabies and died of some other disease only.
Now, my advice for you would be to complete the remaining 2 doses.
There is no definitive answer to all your questions as these are all theoretical questions and no one can give you 100% definitive answers.
What is important now is your health.
PEP has it's limitations but it also helps you to prevent rabies.
Think that why these vaccines are given, if they are not effective.
So, whatever be the stage of rabies, these vaccines help you to prevent them.
So, taking all 5 doses is the most effective way to prevent rabies, it is the right approach at this time as.

Is there any risk? D dog showed symptoms after 24 days and died after 28 days of exposure? Is der any chance that d dog was diseased and shedding virus at the tym of the exposure?can a dog with rabies after shedding virus in its saliva and showing signs live up to 28 days? Can a rabid dog transmit virus in its incubation period? All these muscle twitches dat I am getting on my body n d sensation that I am having at d site is it related to rabies or m I thinking too much? U din't get my question. I already completed my 5 doses. I was asking about d 6th dose at 90 days. Should I take that?

Dear Friend, A dog can shed virus in the incubation period as well.
However a dog can't live beyond 10 days if it has rabies ongoing.
Since you have been previously vaccinated you must be having antibodies against the rabies virus so these injections would have booster up your immunity. So if we see the whole picture, we see following:
1. The exposure was extremely low risk with no fluid contact.
2. The dog didn't die with in 10 days. However it died later.
3. You were previously vaccinated and the antibodies gives protection for up to 10 years.

So, seeing all this there are no chances of rabies.
The muscle twitchings that you are getting are benign in nature and do occur as a result of anxiety.

How is your fever now??
