Based on your description, I feel the possibility of vitreous floater. These are perfectly harmless opacities in a fluid at the back of eye with gelly like consistency. That is the reason, they move around with the movement of the eye, of course with a fraction of a second's time lag. They are more visible on a white background.
These opacities do not respond to treatment usually because that area does not have any blood supply. so any oral medication will not reach there easily. It is vit. C that is usually very helpful in these cases. Other than natural sources, 500 mg. of vit. C can be taken for few weeks.
These opacities are best ignored, as following them does not help.
Regarding optic nerve thickness, it is absolutely normal. The thickness of optic nerve you are mentioning is at times normal. Thin retina as long as eye pressure is normal, is not the matter to be worried about. If you have normal eye pressure, then the best is to have a yearly follow up just to be on safer side.
Scratching of eye do not cause the floaters. Also your history of hit over the nose looks insignificant.
You do not have a life threatening problem and as a treatment, all you can have is vit c supplementation.
