Hi,
I took doxycycline three weeks back. Immediately after that i had severare gastritis issue.
Please note that i had been suffering from GERD past 6 months .
Is doxycycline the right line of treatment for follicities ? is there any alternative i can take that wont cause gastritis issue.
The tablet edox dt also contains sacchamyces boulardi. does this have any interaction with nexium

Brief Answer:
Recurrent folliculitis
Detailed Answer:
Hi.
Doxycycline is a broad spectrum antibiotic and is not the first choice for a simple case of folliculitis. Doxycycline may cause gastritis.
A penicillin group of drug is first line empirical choice of antibiotic for simple bacterial folliculitis.
However, recurrent scalp folliculitis is a different and difficult proposition to handle for the simple reason that it is recurrent
It could be due to a fungal etiology (oily scalp with dandruff) Or may be due to staph. aureus bacterial colonization of the nasal flora, giving rise to recurrent bacterial infection in the head and neck region.
I would suggest you to use an antifungal shampoo, thrice daily (2% nizral solution).
I would have prescribed a different antibiotic e.g either from the penicillin group (Augmentin) OR from cephalosporin group (Cephalexin or cefadroxil) for a week.
An OTC topical antibacterial ointment e.g 2% mupirocin ointment should be applied to the folliculitis locally as well as to the nasal vestibule (just inside the nares) twice daily for 5 days to eradicate carrier state.
Saccharomyces does'nt interact with Nexium.

Hi,
I am having the follicities issue past 6 months.
I have seen 3 different doctors so far. The only one that worked so far is e dox dt. But as soon as i stop using it, in a week i get back the follicities.
So should I stop taking this one and start taking Augmentin ?
I need a tablet that will not cause gastritis. As gastritis leads to severe issue where i cant even eat. it gets so worse. It took two week for me to recover and get back into eating.
The follicities is equally worse as its very very itchy and painful in the head. So could you help me with the best line of approach?
I am taking anti fungal shampoo past a month.

To add a note,
I dook E dox DT for a week and i stopped 3 weeks back. Since I had gastritis issue i didn take it.
Since last week the boils came back and its really annoying now.
I just took the e dox dt tablet again today. Will stopping the anti biotics after taking once or taking them with intervals will cause any issue?
Can I stop taking this and take something else that wont cause gastritis...

Hi.
I would suggest a different antibiotic e.g either from the penicillin group (Augmentin 625 mg thrice daily) OR from cephalosporin group (Cefadroxil 500 mg twice daily e.g Droxyl tablet) for a week.
An OTC topical antibacterial ointment e.g 2% mupirocin ointment should be applied to the folliculitis locally as well as to the nasal vestibule (just inside the nares) twice daily for 5 days to eradicate carrier state. You could be a nasal carrier of staph aureus.
Get a blood sugar test to rule out diabetes, if not already performed.

HI,
Please see http://canlyme.com/2013/08/28/the-combined-use-of-proton-pump-inhibitors-and-antibiotics-as-risk-factors-for-clostridium-difficile-infection/
There seems to be a higher risk between PPI and Antibiotic. Since PPI reduces stomach acid, antiobiotics usage during PPI may lead to c.difficile and etc.
I've done the sugar test and its ruled out.
Does the two antibiotics you suggested not react with PPI? Just making double sure as i am really concerned with the trouble i faced after taking e dox dt.

Hi
Clostridium difficile infection is an altogether different thing. It is in fact more common with broad spectrum antibiotics like doxycycline or clindamycin than with Augmentin or Cefadroxil, since these broad spectrum antibiotics suppress the normal resident flora in the large intestine, specially when taken over a long period of time and as repeated courses.
PPI and antibiotic combined together increase the risk for C. difficile diarrhea but that is totally unrelated to gastritis.
I would say that you can safely take a PPI together with these antibiotics. Augmentin can cause loose stools due to disturbance in resident bacterial flora but it is not associated with C. difficile diarrhea. Out of the two I sugested, I think Droxyl 500 mg, twice daily is a better option for you than augmentin

Hello doctor,
Today I had consulted a dermatologist. She says that the condition is severear since i have over 100 boils in the scalp.
Its very itchy and painful.
She has prescribed taxim - 0 200 (cefixime) - 2 tablets a day
and has given oxiconazole cream to be applied daily
with shaboowash shampoo twice a week with salicylic acid& ketazonazone shampoo once a week.
She also said that she would have given e dox dt as well (doxiciline) but since it triggers gastritis she gave this one.
1. Is this treatment going to be effective ?
The boils are very itchy and painful. Even while i sleep, when it touhes pillow it pains.
2. Will these tablets create any gastritis too?
Should I try what you suggested or what this doctor suggested? I thought physical examination would be better so had to visit this doc today.

Recurrent folliculitis
Detailed Answer:
Hi.
You can take either of the antibiotics (suggested by her or me), however I usually give Oral Penicillins as first choice. Continue for a week. A week of oral antibiotics either Augmentin Or Cefixime wont cause gastritis. Anyways, you should continue with PPI.
Since recurrence is the main issue that has bothered you since long therefore eradication of possible carrier state is also to be done. An OTC topical antibacterial ointment e.g 2% mupirocin ointment should be applied to the folliculitis locally as well as to the nasal vestibule (just inside the nares) twice daily for 5 days to eradicate carrier state.
