Can HIV Be Contracted Through Non-penetrative Sexual Act?

Question: Hi Doctor,
I would like you to assess the following situation for exposure to any STIs (including but not limited to HIV).
A few months ago I went to a massage parlor in Bangkok. No penetrative anal, oral or vaginal sex took place. The masseuse (whose HIV status I do not know) rubbed oil on herself and massaged my penis between her breasts. At some point, she rubbed my penis head (I am not circumcised) against her nipple. Her nipple was smooth and without any cracks and to the best of my knowledge there was no secretion visible on her nipple however for obvious reasons I cannot be 100% certain. My concern is if there were any secretions (such as breast milk) would it be a potential risk for STIs or HIV?

Thanks for posting your query.
You had no penetrative sex.
Your activities, rubbing against breast and nipple have no risk for either HIV or other STDs, even in the presence of any secretion from nipple. Breast secretion has very low viral concentration in a individual with infection. Even in the presence of prolonged sucking for months amounts to the extent of risk up to 25-30 % only. Just a contact with the secretion carry negligible risk for HIV /STD. When there is no external lesion over the breast, STD risk is almost nil.

At the start of your response, you mention that the activity is NO RISK for STD/HIV however at the end you mention NEGLIGIBLE RISK / ALMOST NIL. Could you please point out the difference between the two and confirm what is your final medical opinion on this situation?

Would appreciate if you could answer the following:

1. Based on this situation alone, is any form of testing required.
2. In all your years of dealing with the subject, have you ever come across a case in which HIV transmission occurred through non penetrative activities such as genital rubbing or mammary intercourse

You need no test for HIV/STD.

Nobody can say categorically nil. Negligible and almost nil is equivalent to 'nil' only.
1.In my opinion you need no test for HIV/STD if your activities restricted only to breast and nipple rubbing.
2. I had not come across such a case of HIV transmission, without any penetrative sex and only with rubbing against breast and nipple in my 25 years of HIV practice or in any other journal or conferences.

Since we are on this topic, may I request for your expert opinion on the following situations as well as regards to HIV risk?

1. Receiving oral sex (fellatio) without condom
2. Receiving nipple licking
3. Woman masturbating herself and then using same hand to masturbate penis

All activities mentioned by you (protected oral sex, receiving nipple licking, masturbation with a contaminated hands) have either almost nil risk or negligible risk for HIV infection. Nipple licking has almost nil risk, whereas other activities carry negligible risk for HIV.

Thanks Doctor. My question was receiving oral sex without a condom (and not protected as you have mentioned). I know that would change the risk profile for some STIs but how about for HIV?

I mistakenly read as with condom. Unprotected oral sex carries a low risk as per Centers for Diseases Control, XXXXXXX USA. Anal and vaginal sex has higher risks compared to oral sex. Vaginal intercourse carries a risk of 4-8/10000 penetration. Oral sex is still lower risk.
STDs can be transmitted if the partner has STD.
