Can HIV Be Contracted Through Non-penetrative Sexual Activity?

default
Posted on Tue, 16 Aug 2016
Question: hi doctor, I had a question regarding possible exposure to hiv. Whilst on a business trip in China, I happened to go to a massage parlor. Post the massage, the lady and me made out for a while and i might have kissed her vagina for a 5-10 sec period. We did not have intercourse. Do you think I am at risk?

Also, as an addition, the lady has also given me a blow job whilst using a condom. Do let me know if you require any further information. Looking forward to your response

Hi
Thank you for query.I am Dr Bharatesh D Basti answering your query.
Welcome to health care magic. I have gone through the details.

HIV is not transmitted by massage, kissing the vagina or by protected blow job (even if she is positive). She may be negative too.You need not worry about HIV.
You can allay fears of HIV.

You can get back for any clarification.

Hi Doc Bharatesh

I would still want to undertake some tests to rule this out. Can you help me with the appropriate tests and places where I can get them done in Bangalore?

Is it true that the antobody tests can only be conclusive after 3 months (which is a really long time). Is there any other test possible in XXXXXXX which can help me move beyond this fear and continue my life normally?

Specifically on oral sex, I read up that it does have a very small probability of causing transmission and hence my concern.

Brief Answer:
HIV 1& 2 RNA PCR

Detailed Answer:
Hi
Thank you for follow up query.

HIV is not transmitted by oral sex with HIV positive person unless both have bleeding wounds in the area of contact. It is very unlikely in your scenario. Moreover it is protected so there is no chance of transmission. You need not worry about HIV.
You can allay fears of HIV. I do not see the need for HIV tests.

If you still want to undergo tests, then you can go for
1. HIV 1& 2 RNA PCR post 10-14 days exposure. This will allay your fears. It is highly sensitive (99% sensitive) test after 10 days exposure.
2. HIV1& 2 antibodies test after 3 months post exposure. This will be final conclusive test.

You can get them done in any reputed laboratories like SRL, LalPath lab, Metropolis, XXXXXXX diagnsostics ,Thyrocare or in any corporate hospitals.

You can get back for any clarification.

Hi Dr Bharatesh

One more question, what are the odds or chances(based on past clinical cases) of a male getting HIV whilst giving oral sex to a Female if say the Female is already HIV positive (I am not sure in this case and hence assuming the worst case scenario).

Obviously, there was no protection used in the above act as compared to the reverse scenario. I was more concerned about the above act than the protected oral sex (where the Male is the recipient).

Thanks a lot for your responses so far.

Brief Answer:
Risk is almost nil

Detailed Answer:
Hi
Thank you for follow up query.

The risk of unprotected oral sex with a HIV positive person is too low unless there are bleeding wounds in both in the area of contact. So it is almost nil.
Saliva does not transmit HIV. HIV does not survive in the Gastrointestinal tract .
Unless there are bleeding wounds in both the risk is almost nil.
If there are bleeding wounds,it would have been noticed by you.

You can get back for any clarification.

Hi Doc

Final question, I am seeing mixed opinions on different forums regarding the conclusiveness of the HIV 1& 2 RNA PCR post 10-14 days exposure.

I have seen in this forum that HIV 2 virus can be detected only after 90 days. Could you explain the veracity of these tests in detail so that I can chose accordingly. Also, are there other STDs that I should get myself tested for? If so what are the suitable tests for the same

I am currently planning to do HIV 1& 2 RNA PCR post 12 days of exposure. Pls advise if I should be doing any other test as well to double confirm that I am not afflicted by HIV 1 or 2.

Brief Answer:
Details explained

Detailed Answer:
Hi
Thank you for follow up query.

HIV 1&2 RNA PCR is very sensitive(99%) after 10-12 days exposure. You can rely on it. The chances of tests coming positive later is nil. But to have conclusive results and to be relieved of anxiety, it may be better to go for HIV 1&2 antibodies test after 3 months exposure.

I do not see need for any HIV tests in your scenario.

You can get other STDs tests as a routine like Hepatitis B (HBSAG), VDRL, HSV 1&2( IgMand IgG) antibodies.

You can get back for any clarification.

Hi Doctor

Many thanks for your responses so far. I am still quite worried about the exposure risks in this scenario, even though you have told me multiple times that the statistical probability is near zero.

Are there any early symptoms which may be observed if I have acquired HIV? I somehow am feeling much more weaker/ tired than usual (this may just be me being paranoid).

Do let me know. Also, do you personally know of any cases where oral sex has lead to hiv affliction?

Brief Answer:
Allay fears of HIV.

Detailed Answer:
Hi
Thank you for follow up query.

Yes, the symptoms may be due to anxiety or stress.
Do not worry.Just forget the episode. There is no possibility of HIV transmission.

The symptoms of early HIV is non specific. It may present with fever, diarrhea ,weight loss , cough with expectoration, etc .They can occur in other conditions too.So they are non specific.

I have not seen any case due to protected oral sex.
You can allay fears of HIV.

Hi Doctor

Have you seen or heard of any cases where the male has acquired HIV for giving oral sex to the Female Partner without any protection (such as dental cap and so on).

I shall anyways get the PCR RNA check done once I am in XXXXXXX I have also read that it is advisable to do the PCR RNA test 17 days post the incident. Is there such a rule of thumb or best practise?

Brief Answer:
Sensitive after 10-14 days itself.

Detailed Answer:
Hi
Thank you for follow up query.

I think you are becoming too anxious and thinking of all possiblities which are not applicable to you.

The scenario mentioned by you are theoretically possible and practically rare.
HIV RNA PCR is very sensitive after 10-14 days ,so good after 17 days too.
Do not worry and get it done after 14 days.

You can get back for any clarification.
