Can HIV Be Contracted Through Protected Oral Sex?

Question: Hello doctor I m a 27 year male from XXXXXXX One month before I had protected oral sex with a csw girl in malaysia. During oral sex she licked my scrotum several times. After my exposure I am very much worried about hiv. 7 days from exposure a small red painless dot appeared on my inner thigh and abdomen area. I consulted a dermatologist and explained him about my exposure. He advised me that the rashes look like scabies. He advised me to take a hiv test to relieve from my anxiety. I took a hiv pcr rna real time test on 12 days from exposure and the result came as target not detected. After 10 days from exposure the rashes in thigh and abdomen disappeared and pimples like dots started coming in shoulder and chest area. It started growing in numbers and again I consulted a dermatologist and he gave the opinion that the rashes look like psoriasis or May be due to hormone change. He advised Me to take vdrl test, hepatitis test and hiv test to relieve from my stress. I took hiv pcr rna real time test on 21 days from exposure and the result came as target not detected. I also took the vdrl and hepatitis test and all the result came as negative. I again took a hiv cmia duo test on 25 th day from exposure and the result came as non reactive. After 25 days from my exposure I started to take Isotretinoin tablet for acne as suggested by dermatologist and I took a hiv rapid tridot test in 35 th day from exposure and the result came as negative and again I took a hiv combo test at 37 days from exposure and the result came as non reactive. I again took a hiv rna pcr test @ 48 th day and the result came as target not detected. I also took a hiv duo test, hsv Igm, vdrl test @ 48 th day and all the result came as negative. I also took a cd4 test and my absolute cd4 count is 494 while the normal reference interval is 424 to 1500 In that test. And my cd4 percentage is 30.19 while the normal reference interval is 31 to 59 percent. I m very much stressed after seeing this.
1. What's the reason for my low cd4 count?
2. Is my hiv1 test results conclusive ? What's the reason for my rashes?
3. Will my duo test on 48th dAy detects hiv 2 ?
4. Is my pcr test on 12,21,48 days conclusive regarding hiv 1 ?
5. What are the other acute infections that causes a low cd4 count? Will psychological stress cause a decrease in cd4 count?

Protected oral sex carries vary low risk of HIV or other STDs and doing licking of scrotum is no risk activity for HIV. Negative test results for HIV even at 48th day along with negative HSV, VDRL and Hepatitis also favors the fact that you had not contracted any infection so you need not to worry or get anxious.

Duos test is for HIV 1 but the possibility of HIV 2 is very rare because it is very uncommon in XXXXXXX and found only in certain parts of XXXXXXX HIV RNA PCR test is conclusive after 48 days and your symptoms are mostly due to skin infection or side effect of drug but not related to HIV so don't think much.

1. is my pcr test on 12,21,48 days conclusive regarding hiv negative? Will the viral load Will be more than 40ml in 12 and 21 st day and Will the viral load Will be more than 20 ml in 48 days? because my pcr test limit of detection is 40Ml for 12, 21day and 20ml for 48 days.
2. If the viral load is less at that time Will the antigen be produced which is produced in early stages of hiv infection?in that case my hiv duo test on 25,37,48 day would have detected the antigen. is that correct doctor? in that case Will my hiv duo test is conclusive?
3. I have experienced peripheral neuropathy (i.e.) burning sensation on 7 days after exposure in thighs and at that time I had a small red painless dot in thighs at the same time. Then after that pimple like non itchy rash appeared on chest and shoulder. the pimple like eruptions lasted for 6 weeks and now it went away. what's the reason for this symptoms? Are these symptoms indicates any other infection other than hiv?
4. If the symptoms are related to hiv then the virus will be at detectable level at that time. is that correct ?
5. my blood test on 10th day from exposure also showed a rise in wbc 12.6 I asked my doctor about the reason for high wbc and my doctor explained that the wbc rise is due to my body fighting any acute infection. in that case if my body is fighting hiv infection then the hiv viral load test or hiv duo test will have detected the hiv if I am infected? is that correct doctor? Please answer me doctor regarding my queries . Please clarify me

If you have an infection then it would be detected surely with the tests but it is not the scenario here. All your test results are negative then nothing to be worried about. Allay the fear or anxiety of HIV.

HIV RNA PCR is definitely conclusive after 48 days. There are no specific symptoms of HIV because you are free from infection. Peripheral neuropathy is due to some vitamin deficiency not due to HIV.

High WBC count is very usual in common infection also so don't get bothered about it. You can surely consider yourself absolutely risk free from HIV. Be relaxed and move ahead in life happily.

2. is hiv 1/2 ag ab combo test able to detect hiv2 in 25,37,48 days period? is there any chance of hiv2 from protected oral sex? 
3. have u seen or is there any possibility of negative result of hiv rna and hiv ag ab combo test after 6 weeks turning positive later? 
can I take my test result as conclusive and move forward with my family life. do I need further hiv test?

HIV RNA and Duos test is conclusive after 48 days. It is not possible to get positive result after being negative at 6 weeks through HIV RNA and Ab-Ag combo test so don't get worried or anxious.

HIV2 is very uncommon in XXXXXXX and found only in some parts of XXXXXXX and if present It would have been detected by HIV1/2 Ag-Ab combo test. Just calm down as your test result are conclusive.

1. My differential blood test on 9th day from exposure has lymph percentage of 32.8 and lym value is 4.1 and the test on 35 th day has a value lymph percentage 45 percent and lym value 3.8. What's the reason for this if lymph percentage is low then lym value is high and if lym value decrease then the lymph percentage is high ? Does this indicates any infection or is it abnormal?
2. I have read in internet that hiv duo test Will detect antigen and antibodies. Will the duo test detects both hiv1 and hiv2 antigen? 
3. If the duo test detects only hiv1 antigen and if there is only hiv2 infection and Will the test cross reacts and show positive? 
4. Will there be enough antibodies at 48 days for hiv 2 infection?
5. I took a real time pcr for hiv 1 @ 12,21,48 days and all the reason came as negative. if there is hiv2 Will the pcr test I have took shows some viral load ?

The lymphocyte count is normal so infection is not there. HIV Duos test detects both antigen, antibody for HIV1 and HIV2 is very uncommon in XXXXXXX then the possibility of cross reaction is also ruled out.

After 6 weeks antibodies against HIV virus will fully develop so the result is considered highly reliable and RNA PCR is also negative then nothing to fear about because you are absolutely safe from HIV.

Thanks doctor for your convincing reply. I did a hiv2 real time pcr test at 48 days and the result came as target not detected. limit of quantification of the test is 900 copies per ML. Today I have received the result and how far is my test result conclusive?
1. can I be sure that I am negative for both hiv 1 and hiv 2.
2. Will there be enough viral copies (I.e) 900 copies at 48 days?
3. If the viral load is less than 900 copies then there will be antibodies. is that correct doctor? can I conclude that I m not infected with hiv 1 and 2? Please clarify doctor.
