Can HIV Be Contracted Through Protected Vaginal Intercourse?

default
Posted on Tue, 2 Feb 2016
Question: Hello...i had 2 protected vaginal intercourse with 2 different female escorts on 9 Dec. Condom was used end to end. On 14 Dec I did a HIV ag/ab cmia test which came out as negative. The escorts were appearing professional and do not even allow you to kiss them.

Am terribly worried. Am I at risk? Is a cmia test at 5 days valid?
doctor
Answered by Dr. Sunil Jangid (1 hour later)
Brief Answer:
The risk of getting HIV is not there in your case so don't worry.

Detailed Answer:
Dear XXXX,

Welcome to HCM.

You had protected vaginal intercourse with two different escorts than no need to worry as the risk of getting HIV with protected sex is nil.

The CMIA HIV Ag/Ab test at 5 days is quite reliable but you have to go for repeat testing after 1 month with 4th generation HIV Duos or CMIA test to be completely sure about your status.

Even though the escorts were appearing clean and professional but having multiple sexual partners is high risk for HIV so I will advice you as your friend to stay away from such unsafe sexual practices to prevent the future risk of HIV and other STDs.

Thank You

Dear doctor. Thank you for your encouraging words. Yes I will never resort to this again. Have not slept peacefully ever since then.

I have a few questions:

- How reliable is the CMIA ag/ab test at 5 days?

- Today is 12 days post exposure and I don't have any ARS symptoms. Are they likely to occur?

- The condom seemed ok. How can we know if there is hole or it has broken? I am hearing that condoms break often. Durex was used. I did not touch the condom. After the session the escort removed it. I am also not circumcised.

Please help doctor am terribly worried. Thanks..
doctor
Answered by Dr. Sunil Jangid (3 hours later)
Brief Answer:
ARS symptoms usually take at least 2 weeks to develop after the infection.

Detailed Answer:
Dear XXXX,

The CMIA Ag/Ab test at 5 days is not highly reliable and according to international HIV guidelines to get 100% accurate and conclusive report you have to get tested after 3 months of exposure. The opinion regarding this issue differ from physician to physician, some consider 6 weeks test result as conclusive.

Acute Retro-viral Syndrome symptoms usually take at-least 2 weeks to develop as after infection occurs. The user can only know whether the condom was broken or not and as per your detail I think it was intact at the end. Don't think much about HIV and no need to worry because you are absolutely safe.

Thank you once again doctor.

You are saying the cmia ag/ab test is quite reliable earlier so am confused over this.

Under what basis you feel my fear is unnecessary? Should I test again? Am so scared doctor.

Also is cmia same as 4th Gen duo?
doctor
Answered by Dr. Sunil Jangid (48 minutes later)
Brief Answer:
You had protected sex with both escorts that's why your fear is unnecessary

Detailed Answer:
Dear XXXX,

You had protected sexual intercourse with both the escorts than the chances of an infection are already not there and that is why your fear is unnecessary.

Both CMIA and 4th generation Duos are same detecting both HIV antibody and P24 antigen. As such no need to get tested again but for your mental satisfaction go for repeat testing after one month which will clear all your doubts. Don't get feared so much as the result will surely come in your favor.

Thank You

Thank you once again doctor for your encouraging words.

Last night I had a common cold/runny nose. Would this be a symptom? I feel alright this morning.

Also, the test is not required any further as per your advice correct? Please advice.

Is it safe for me to proceed with conjugal relationship with my wife?
doctor
Answered by Dr. Sunil Jangid (3 hours later)
Brief Answer:
The common cold or running nose is not the symptom of HIV so don't worry.

Detailed Answer:
Dear XXXX,

The common cold or running nose is very common problem during season and it is due to some other viral infection not related to HIV so don't get worried so much.

As such no need of any test further but you are so anxious or tensed then better to get tested after one month with 4th generation test which result will clear all your doubts and give you mental satisfaction or relief from stress.

You had protected sex with both the escorts than the chances of getting HIV are already not there and you can definitely proceed in conjugal relation with your wife. Just calm down and move ahead in life happily.

Hello doctor thank you once again for your feedback.

Today I went to a GP for my cold and also shared the same message i shared with you. He has taken all the std and HIV test. The HIV test he has taken is Elisa.

I have a few queries:

- how accurate is Elisa compared to the cmia I took 5 days after exposure. Test for Elisa taken today is 14 days after. I am yet to receive results and this is making me so worried.

- 14 days passed without any symptoms. Just anxiety and stress. Would std other than HIV show symptoms in 14 days?

- is condom always safe even if we assume the escort is hiv+?

Am getting really worried
doctor
Answered by Dr. Sunil Jangid (45 minutes later)
Brief Answer:
ELISA test taken after 14 days exposure is reliable but not 100% accurate.

Detailed Answer:
Dear XXXX,

ELISA is an antibody test for HIV and the production of antibodies varies from 15 days to 3 months which is known as window period that is why ELISA test taken at 14 days of exposure is reliable but not 100% accurate and to be repeated after 3 months to be completely sure about your status.

The use of condom is always safe when doing intercourse with an escort but it is high risk for HIV or other STDs so better to avoid such unsafe practices. Just relax, avoid excessive stress or anxiety and no need to worry so much because you are not at risk of getting HIV. Wishing you happy and healthy life ahead.

Thank you doctor.

Will you foresee any difference in report of my cmia vs Elisa?

Should std symptoms show up by now if there are?
doctor
Answered by Dr. Sunil Jangid (21 minutes later)
Brief Answer:
STD symptoms will appear if infection is present so don't worry about that.

Detailed Answer:
Dear XXXX,

STD symptoms will definitely appear if infection is present and you have not any symptoms so don't worry because you are risk free. I don't think that the report of ELISA would be different from CMIA so be relaxed and move ahead in life happily.

Thank you doctor. Occasionally am also having on and off pain under my right armpit is not severe but on and off. Is this a worrying symptom?

Also, 15 days pass exposure would body show any reaction to possible HIV infection by now? Based on your experience with your customers.

I know you feel am safe. Is just am still having the fear. Has there been any cases someone infected with even condom intact and not broken?
doctor
Answered by Dr. Sunil Jangid (44 minutes later)
Brief Answer:
The pain under right armpit is not worrying symptom for HIV.

Detailed Answer:
Dear XXXX,

The on and off pain under right armpit is not at all worrying symptom of HIV, it is due to muscle spam or any other reason for which you can take some pain killer like brufen or aceclofenac so don't get feared so much.

I have not seen any case who has been infected with even though the condom was intact through out the act. Don't take unnecessary stress or tension because this is also not good for your health. Be relaxed and move ahead in life.

Hello doctor. Thanks for your feedback. Occasionally I get tingling sensation in my scrotum. Is this a risk? Otherwise I am not having any other symptoms.

Do you see a real need for me to take a cmia test in months time since condom is safe?
doctor
Answered by Dr. Sunil Jangid (2 hours later)
Brief Answer:
You need to get tested after 3 months as per HIV testing protocol.

Detailed Answer:
Dear XXXX,

Occasional tingling sensation in scrotum is not the worrying symptom for HIV so don't get worried as the risk is already not there in your case.

The condom is safe but according to international HIV guidelines you need to be tested after 3 month to be 100% sure about your status nothing else so be relaxed and get tested as per protocol.

Dear doctor, thanks for the feedback.

When you say the Elisa ag/ab HIV 1 and 2 test is reliable. How reliable is this? In 14 days should this already be detecting any antigen?

Today marks 28 days past exposure. Although you are saying risk is extremely low am just very worried.
doctor
Answered by Dr. Sunil Jangid (25 minutes later)
Brief Answer:
ELISA test is for detection of antibodies against HIV not antigen.

Detailed Answer:
Dear XXXX,

ELISA test is for detection of antibodies against HIV not antigen and it is not highly reliable at 14 days but you had protected sex both the times that is why the chances of transmission are not there.

Be relaxed and avoid thinking too much because it is also not good for your health. Get tested after 3 months and the result will surely come in your favor. Don't get so much worried or anxious.

Hello doctor. Thanks for reply.

In my report it shows as

HIV 1 and 2 AG/AB (ELISA) as Non Reactive.

Does this mean they tested antigen as well?
doctor
Answered by Dr. Sunil Jangid (15 minutes later)
Brief Answer:
ELISA is for testing antibodies usually so I don't think they have tested for antigen

Detailed Answer:
Dear XXXX,

Welcome back again.

ELISA is usually detection of antibodies so I don't think they have tested for antigen. You can confirm it with lab people whether tested for antigen or not.

Again I will say you are taking unnecessary stress which is not beneficial for you so avoid thinking so much and stay relaxed as you are not at risk.
