Question: Doctor I had unprotected sex with csw on March 12th. During sex condom broke and it lasts for 1min. I am sure I haven't ejaculated inside her. I took tests on 6th day, 14th day and 32nd on hiv Elisa 3rd generation. Also done Hiv proviral DNA pcr test on 10th day in reliance life sciences lab, XXXXXXX and in that report it mentioned as Windows period for this test is only 48-72 hours. Since pcr and Elisa on 32nd day is good indicator on my hiv status or still I need to be in fear. Pls help me as my anxiety kills

Brief Answer:
You need to go for repeat testing after 3 months.

Detailed Answer:
Dear XXXX,

Welcome to HCM.

You had unprotected sex with CSW and condom is broken which last for one minute than the risk of getting HIV is very unlikely with such short single exposure even though she was HIV positive.

You took HIV ELISA 3rd generation tests on 3rd, 10 and 32nd day along with DNA PCR at 10th day and all are negative then nothing to worry as these test are considered highly reliable after one month.

The window period for HIV varies from 15days to 3 months so you need to go for repeat testing after 3 months as per routine HIV guidelines to get 100% accurate and conclusive result. Don't get anxious or worried as the risk is very unlikely in your case.

Hope this info will help you. If still any doubt left in your mind feel fee to ask me and I am always here to help you any time. Stay happy and healthy.

Thank You

Hi Dr XXXXXXX
Thanks a lot for your response. I have few questions to ask.
1. How much percentage I am out of hiv?
2. I have attached my pcr report and pls see that it mentioned as Window period is from 48-72 hours. It's proviral test. I took on 10th day and the counselor at YrG center told me we can believe this test 99%.
3. Also I read somewhere that Elisa after 4 weeks also 95% accuracy.
Please help me as I am really under too much fear of symptoms facing. I am thinking as whatever I am facing is relating with ARS. I will take test for sure after 8 and 12 weeks. I just want to understand how much I am out of serious situation.

Brief Answer:
You are almost 100% free from HIV.

Detailed Answer:
Dear XXXX,

Welcome back and thanks for follow-up.

You had rightly said that DNA PCR at 10th day and ELISA after one month are considered accurate as also said by the counsellor so you are almost 100 percent safe from HIV but still to declare someone absolutely free from HIV it is mandatory to get the test done after 3 months of last exposure.

It is written in the report that the window period is reduced to 48-72 hrs by DNA PCR as it is able to detect the presence of virus in early period of the infection even after 10 days also but the window period ranges from 15 days to 3 months and that is why 3 months testing is compulsory for HIV.

Don't get feared about the symptoms of ARS as it is very unlikely or practically not possible in your case so be relaxed and move ahead in life as there is nothing serious at all. If still any doubt or query left you can ask me any time without hesitation.

Thank You

Thanks a lot doctor. I would have produced antibodies by this time know. . I am a diabetic for past 2 years. Will diabetic cause antibodies to produce late? I am facing symptoms like skin rashes and throat pain with tonsils and severe neck pain. Do this symptoms have anything related to hiv. I am thinking to pcr again after 5 weeks? Will that be helpful for me in my case

Brief Answer:
The symptoms are not related to HIV.

Detailed Answer:
Dear XXXX,

Welcome back once again.

The past history of diabetes has not any relation to production of HIV antibodies so don't worry about that and the symptoms you have are mostly due to other infection but not related to HIV in any way.

As such no test is required at 5 weeks but for your satisfaction you can go for DNA PCR again which negative result will give you sense of relief. Don't get tensed or feared as the risk is very unlikely.

Thank You

Thanks a lot doctor for your answer. So as based on my test result pcr and Elisa I can be with no problems. Right. Apart from medical recommendation, I believe my tests are enough as per ur suggestions. I will take one more test after 8 week and 12 week. Taking tests at 8 week also good indicator know doctor?

My only question is how my pcr test report says window period is 48-72 hours only. Also they suggested this tests for child also. So I can believe this tests right? Since the reliance is a big research team working for lab in XXXXXXX They won't put this window period just like that know. Also YrG is a big aids Institute having 21k patients and they told me to believe this pcr test. What is ur tot on this pcr test doctor. I am sorry to trouble u again but this is my last question. I am in so much of worry doctor. That's why asking too many questions

Brief Answer:
PCR test is reliable and accurate.

Detailed Answer:
Dear XXXX,

It is mentioned in the report that the window period is reduced to 48-72 hrs. They didn't mention that it is 48-72 hrs so go through it carefully and don't get worried about it. PCR test is also reliable and you can definitely trust the results of the test.

Avoid thinking too much about HIV. Do meditation and yoga for mental relaxation and to alleviate the anxiety. Go for repeat testing after 8 and 12 weeks and the result will surely come in your favour.
