Brief Answer:
Cardiac evalution should be done to rule out angina.

Detailed Answer:
Hello Dear,
Thanks for query.
As per your question no most likely not.
Omeprazole infact is used in gastritis which causes pain in chest area.
I would suggest you to go for few cardiac tests like EKG and 2d echo to rule out cardiac involvement as pain is refractory.
Hope i answered your query well.
Thank you!

Hi again,
Yes cardiac pain can be felt in right side also.
Its a myth that cardiac pain is limited to left side only.

Follow up with investigations.

Detailed Answer:
Hi again,
Yes most likely all symptoms are because of GERD.
Gastritis and GERD can mimic all symptoms of cardiac problem hence i would suggest you to take antacid along with omeprazole.
An upper G.I endoscopy can also be planned to rule out the cause of acid formation.
suggestion
Better to go for a 2d echo and start taking antacid with omeprazole.

Wishing you a quick recovery.
Regards.

Detailed Answer:
Hello again,
In case pain does not respond to them extensive cardiac investigation should be done to rule out possibilities of angina(cardiac pain).

If pain is due to inflammation caused by Esophagitis it would definitely respond to bit high dose PPI's with Antacid(Mucaine gel or same composition preferably).

Follow up after 5 days.
Thanks.

Detailed Answer:
Hello dear,
Not at all i am really pleased to answer your queries.
See the basic problem is that cardiac, gastric and muscular pain usually mimic each other .Hence just on the basis of location or nature of pain its very difficult to differenciate the cause of pain.
Investigation should be done and each system should be ruled out to reach the final diagnosis.
Aa you have previous history of esophagitis and heartburn most likely apparent cause in your case is gastric.
However only investigations can help us to rule out cardiac or muscular involvement.

Thank you and I would love to give you any other professional advice.

thank you so much. I did not know that esophagitis can cause this pain I have in my chest. It seems like it shifts all over my chest. Like I said , I take 40mg of omeprazole. I also have 20mg which I don't take. Should I take that later in the day too? also, I smoke about 10 cigarettes a day which I know isn't helping. So hard to quit. Thanks for being so patient.

Detailed Answer:
Hi there,
Sorry for the delayed reply.

Yes, esophagitis, Gastritis, esophageal spasm can cause pain in chest, in fact, it can mimic cardiac pain. Therefore thorough investigations are required to diagnose the root cause of your problem.

Yes, you can increase the dose of omeprazole rather start taking Rabeprazole in combination with levosulpiride(prescription drug). It's far more effective than omeprazole.

Chronic smoking increases the risk of cardiac disease by 15 times.
A thorough cardiac evaluation should be done initially.

Detailed Answer:
Hi again.

No Motrin will deteriorate your problem. All anti-inflammatory can cause severe gastritis and can worsen such symptoms.

Advice:
If required take a combination of Acetaminophen and tramadol(Ultracet).
Avoid taking all nonsteroidal anti-inflammatory like ibuprofen, aceclofenac etc.

Don't worry it's a pleasure to help you.
Hope you get well soon.
Best of luck
