GERD or gastritis could cause all this clinical scenario.
From the other hand, anxiety seems to play an important role in your symptomatology.
I recommend trying an tablet of Xanax 0.25mg, and a tablet of omeprazole 20mg. If your symptoms are relieved by such drugs, there is no need to go to the ER.

Coming to this point, I would recommend going to the ER for a physical exam and some tests:
- a resting ECG and cardiac enzymes
- chest X ray study
- complete blood count, PCR, sedimentation rate for inflammation.
Even though repeated ECG have resulted normal, I would recommend performing a cardiac stress test in the future to exclude coronary artery disease.
