Question : I am allergic to tetanus--long family history of allergy. I stepped on a paperclip. I am on Clarithromycin right now for bronchitis. Do I need a tetanus shot?

No. If there is a established family history pf allergy to tetanus, do not get vaccinated.
The Tetanus toxoid is very immunogenic and can cause serious reactions if taken.
Since you are on antibiotics you are protected from various bacterial infections.
