Posted on Mon, 1 Dec 2014
Question: I am of age 78 yrs, weight 73 kg (160 lb), height 162 cm (5ft 4 in).
I am diabetic for the past 35 yrs, a cardiac patient, had undergone coronary bypass (quadruple) in 2001 (at the time of 9/11 WTC attack), a femoral bypass in 2006, angioplasty in 2010, hemoturia in 2011. Have been on warfarin (5-7 mg) ever since, my INR around 1.5 - 2.0. I have enlarged prostate, but not yet undergone any operation, on finast and urimax for the past 15 yrs, ... Other medications include diabetic (oral as well as insulin), ecospirin, monotrate, betaloc,....
I have gastric disorder - difficulty in passing stools and/or loose motion (alternately!), hyperacidity which I find very troublesome. Is it okay for me to take Cyra - D? Any other medication? (I am already on too many medicines - about a dozen a day, and not very keen on taking any more). Please advise!

Many thanks in advance!

Brief Answer:
Yes, you can take Cyra-D.

Detailed Answer:
Hi XXXXXXX,
Thanks for asking.

I am Dr. Prakash HM and I will be answering your query.

Based on your query, my opinion is as follows:

1. Yes, for hyperaciditiy and reflux esophagus disease with heart burn, Cyra-D will be very helpful. You can use it and at present, you need to, to reduce gastritis pain. You might not need to take it long, possibly for 15 days to a month.

2. Most of the medications you are taking are related to either cardiac, or diabetes or prostate. Due to these medications mainly, you could be having drugs induced hyperacidity. Omeprazole should have worked, however as it as not worked, rabeprazole, a newer drug is being used.

3. I am not sure about your drug schedule. Avoid taking any medication on empty stomach. Atleast have a cold milk or some food, before taking medication.

4. Due to your diabetes, possible autonomic system dysfunction along with gastritis, is leading to hypermotility and hypomotility. Try to increase fiber in diet, increase fluid intake (if no severe prostate problems) and always try moderate exercises (even walking). Fiber should be insoluble fibers from fruits or vegetables to avoid constipation. The fluids should help in softening of stools.

5. You would already be taking 5-6 meals minimum per day. Continue the same to avoid gastric stasis. Always move around after a meal. Avoid lying down. Even after medications, move around.

6. For gastritis, reduce stress. Avoid spicy or sour food. Continue medication. Eat moderately with nutritional balanced diet.

7. Do not stop any medications unless advised to.

8. Hope you have got a colonoscopy done. Do get it done every five years minimum at least. Change in bowel habits or blood in stools are ominous signs.

9. Control blood sugar within normal range. Get HbA1C every 3-4 months once to check your blood sugar is within normal range. Also get hemogram to check you have no anemia.

Hope it helps.
Any further queries, happy to help again.
