Brief Answer:
It can cause blood collection or Haematoma

Detailed Answer:
Hello,
I have studied your case. When a patient is on coumadin he should avoid deep intramuscular injections as there is chances of haematoma and infection. It is more common as your age group and Diabetic patients. As rotator cuff injections are given in joint they are usually safe. Still I would suggest you to keep a watch on any unusual pain or swelling in the shoulder or any fever. In case there is such symptoms you need to get ultrasound examination to confirm if there is any blood collection.
I hope this answer will be useful for you.Let me know if there is any other followup questions.
Thanks
