Question: i am XXXXXXX XXXXXXX from XXXXXXX i coach students for getting admissions in IIT and various medical colleges in XXXXXXX from last 7 years .i remain in standing position for 6 to 7 hour daily while teaching 2 years back i suddenly felt in class while speaking tha t now i am going to fall and dont have balance and control on my legs with restlesness with unconsciousness for a moment followed by sweating , when i teaches on plateforms of height the above problem comes rigorously when come down suddenly disappears and i feel comfortable ,while teaching i fear to turn myself it appears i fell down ,feel tightness in neck even in some classroom i feel symptoms in high in some class rooms nothing when two year back i consulted dr kk XXXXXXX XXXXXXX he suggested me to take paxidep cr 12.5 daily and till now i am taking that medicine ,if i stop it again symptoms starts coming i also consulted neurologist they did mri of brain but did not found any abnormalities everything was normal.please suggest the way to come out of this .....is it curable...i want to come out of it at any cost

From the history it appear that you had Panic attack 2 years back while teaching. Panic is extreme severe attack of anxiety, it may associated with dizziness (loss of balance), body tremors, palpitation, muscle spasms (like body tightness), sweating, restlessness and fear of losing control/heart attack/choking.

After panic patient used to have fear of developing such episode in future along with anxiety symptoms.

Your case is having similar presentation. Paxidep is a good antianxiety medicine. If you are having relapse of anxiety after stopping it, then it is better to continue it under your current psychiatrist supervision. In recurrent anxiety cases treatment is required for long term (for years also).

In case of anxiety treatment is highly individualized. Treatment may require modification in future depending on your condition. So continue follow up with current psychiatrist.

Apart from that following Positive lifestyle changes will help along with Paxidep:
-Fix sleep and wake up time, No day time sleep
-Switch off TV/Mobile/Net 30min before sleep
-Start daily Morning brisk walk for 30 minutes.
-Avoid excessive caffeine (tea/coffee/cold drinks). Not more than 2 cups/day.
-Avoid any substance use (if present).
