streptococcal pharyngitis (or tonsillitis) is not supposed to cause chest pain and this kind of pain cannot be attributed to it. Esophagitis sounds likely in your case because hiatal hernias predispose to acid reflux and reflux predisposes to esophagitis. Another potential cause is the following. Antibiotic treatment may cause (as a side effect) yeast infection in the esophagus. In such cases there is - usually - oropharyngeal evidence of candida (milky appearance of the mucosa). Your doctor may detect these signs and advice you accordingly.
If the symptoms have started concurrently with the sore throat then reflection of the pain can be considered if the main site of pain is the throat and chest and back are less painful. This is not common though. Ear pain is far more common in cases of streptococcal pharyngitis.
