Diagnosed With UTI.Burning Sensation In Legs And Back. Which Specialist Should I Visit? What Test To Be Done?

- On 7th of April, I suddenly felt bloated and weakened. When I’ve tried to eliminate stool, I felt a pain in the lower abdomen, right side. In the same day, I had a confirmed urinary infection with E-coli, which was successfully treated with Augmentin.
- Two weeks later I had the same infection, treated again successfully with Augmentin, however, I was left with a pain in the lower abdomen, right side and a mild numbness in the right leg when I was bringing the knee up.
- I mostly felt the pain when I was walking or sitting on a chair, but not when I was lying down.
- Because the pain was getting stronger, I’ve seen a urologist and a general surgeon where I’ve done various tests and all came back negative (CT-Scan, RMN, Ultrasound, Cystoscopy, Colonoscopy)
- One week after finishing the third course of Augmentin, which was prescribed by the urologist, my liver results in the blood were abnormal and I repeated the test 3 days later. One value was still increasing and one was decreasing. I’ve repeated the blood test today, after more than 1 month and the bilirubin has increased again, at 62 but everything else looks normal.
- However, now, 3 months after the first infection, I continue to feel horrible all time and getting even worst, with burning sensation in my right leg and in the back. Also, if I am walking for more than a few minutes, I am experiencing cold feet and numbness in the feet and also extreme pain in the lower abdomen, right side. I am literally lying in bed all day, trying to do no effort at all. However, none of the symptoms are improving. On the contrary.
I’ve now been referred to a neurologist because of the burning sensation and tinglings in the legs, however I believe that there is something wrong in that lower right side of the abdomen, as it all started from there, and then I could feel the pain going up, and then the numbness in the feet and then random aches in the body and abdomen muscles.
- I would like to mention that it doesn’t hurt when I press on the abdomen, only when I am moving or walking and that I haven’t really experienced any constipation or diarrhea, only after the first infection I had 2 days of constipation and then one week of diarheea. (doctors said it might have been from the antibiotics). There is also no fever at all.
Please advise what specialist I should see or what tests would you recommend as I am starting a feel a bit desperate right now because I feel I am getting sicker with each day. The gastroenterologist that I’m seeing now insisted to wait two more weeks to see how it progresses, however I feel worse with each day.
What tests would you recommend that I should do, in addition to what I’ve done? What could be the cause of this?
Thanks for posting your query.
1) Kindly answer the following details:
a) Have you any swelling in the right lower limb?
b) Have you undergone urine culture & sensitivity test?
c) Have you undergone color doppler study of right lower limb (both venous &
arterial)?
2) As you have already gone through all investigation such as colonoscopy, CT scan, cystoscopy and ultrasound, there is least possibility of affection of these systems.
3) There may be possibility of infectious arthritis after acute diarrhea as you have difficulty in walking.
4) Presence of cold extremities, numbness and pain with movement of leg indicates some sort of decreased blood supply in lower limb and may be blockade of venous/ arterial system.
5) You should again go for urine culture & sensitivity (if previously done), although urinary infection doesn't cause right lower limb symptoms.
Put your query if you have.
Best regards,
Dr. Mayank Bhargava

a) There has been no swelling
b) There has been a urine test and culture and it was E-coli when I had the infections. Since then, there seems to be no problem with my urine, as I've repeated the test several times.
c) No doppler test was done
I would like to reiterate the fact that all the symptoms are very random. I had abdominal muscle aches for up until yesterday and since yesterday I have strong pain in my back, which is very focused in one point. (right side, under the shoulder)
Also, yesterday I went to repeat blood test and I had cold feet all day. However, when I returned home and laid down, the right foot and right side was burning again. (up until the liver level). But sometimes only the leg is burning.
I didn't have acute diarrhea and I don't think it's arthritis as I only feel that right joint weakened because of the abdomen pain.
It's even hard for me summarize these symptoms, however, I am sure that everything started from the lower right side of the abdomen as In the beginning the pain was only located there.
On the inside, I always feel a burning pain, moderate but continuous. And the centre of that is definitely that lower right point in the abdomen. However, after the scans, the doctor said that there is nothing surgical showing up on the scans.
I am really confused of what to do next. Please advise what could be the cause of this or where should I go for this.
Thank you
Thanks for sharing your query.
Presence of pain on right back just below the shoulder indicates pain at renal angle with presence of renal disease.
There may be possibilities of muscular pain or neuropathic pain.
Presence of cold extremities, numbness and pain in right lower limb indicates blockade of blood vessels.
You should go for color doppler study of lower limb.
Hope this will solve your query and put your query if you have.
