The most likely scenarios are either a bruise resulting from the abnormality around the scrotum from where some blood vessel that travels to to the head of the penis is present. Blood vessels that supply the head of the penis pass from around the scrotum region, and due to the inflammation and so on, it could have compromised the blood supply (partially) to the head of the penis resulting in the bruise like appearance around the head of the penis.
Or as a side effect to the cream, if it was used before the bruise around the head of the penis appeared.
