Do The Following Ultrasound And Blood Test Reports Suggest Ectopic Pregnancy?

default
Posted on Thu, 16 Mar 2017
Question: Hi,

We have got the ultrasound and blood test done for pregnancy, it showed sac size .26cm seen in midcavity, and HCG BETA is 733(on 20th Feb) which was 463(on 18th Feb), 2 days ago.

Last mesuration cycle ended on 14th XXXXXXX

Most probably this resulted from unprotected intercourse on 26th XXXXXXX

In any case we dont want to proceed with this and want to terminate asap.

We have visited doctors with the reports
Doctor said
-Its not sure that where is the pregnancy, and until HCG BETA count doesnt cross 1500 its hard to say and reflect in ultrasound report.

- There is chance of Ectopic pregnancy.

We have several questions

As We are running out of time because she has to travel out of XXXXXXX for 2 months and only left with 4 days in XXXXXXX

Is it normal progress on HCG BETA count?

Is there any possibility of Ectopic pregnancy?

What should be our next step to terminate this progress asap with less consequences.

Brief Answer:
Ectopic pregnancy should be ruled out

Detailed Answer:
Hi dear,
I have gone through your question and understand your concerns.
No, this is not the normal progress of beta hCG, In a normal intrauterine pregnancy, the beta hCG increases more than double in 48 hours.
If the increase is less than this, then there are chances of ectopic pregnancy.
A transvaginal ultrasound should be done to rule out any ectopic pregnancy.
If there is no ectopic pregnancy, then termination of pregnancy can be done by medical method.
If ectopic pregnancy is there, then it needs to be terminated by medical or surgical method.
Hope you found the answer helpful.

We have got transvaginal u/s done, there its mentioned as sac .26 cm seen in midcavity,
We are planning to have another u/s done day after tomorrow, on the based of those reports, can some medication be provided to terminate the normal/ectopic pregnancy?
Any other suggestions as per your experience are highly appreciated, as I mentioned earlier also that we have time only till friday.

Brief Answer:
Depends on the diagnosis

Detailed Answer:
Hi dear, a repeat transvaginal scan will be confirmatory.
If normal intrauterine pregnancy is there, it can be terminated by tablets mifepristone and misoprostol. However, the abortion process takes 7-10 days and cannot be completed till Friday.
She can for surgical evacuation in that situation, in which the abortion is completed in single setting.
If there is ectopic pregnancy, then it needs to be examined and managed by an expert OBGYN specialist.

Thanks Hi Mam,

Thanks for the guidance, we have got another TVS done and there its identified that its normal intrauterine preganacy.
Now, I see that you have mentioned Mifepriston and Misoprostol tablets for termination.
Kindly guide us on the availability and consumption of the these pills with all precautions that we should keep in mind to avoid any future consequences.

Thanks

Brief Answer:
To be taken under supervision

Detailed Answer:
Hi dear,
Medical abortion using mifepristone and misoprostol should always be taken under the supervision of a registered medical practitioner, so that proper follow up can be done and surgical abortion can be done if the medical fails.
Mifepristone is taken followed by misoprostol after 48 hours.
Abortion process starts within 24 hours of taking misoprostol and bleeding continues for 7-10 days.
Failure of medical abortion or excessive bleeding can occur in 2% cases, in which surgical evacuation should be done as soon as possible.
Hope you found the answer helpful.
