I read that both can prolong QT and cause heart arrhythmia and that there was concern about giving together . Do you often / regularly prescibe them together and have you ever seen any problems ?

They do not have any additive effect on the QT and produce any dangerous arrhythmia.
Both are commonly prescribed medicines and are for different indications. But if both are required for different indications, I have not had any problem where they were prescribed together.Make sure that there is no condition like diarrhoea or vomiting that does not deplete the serum Potassium or that there is no pre-existing QT prolongation.
