I have stopped zapiz 0.125 and inderal la 20 in the morning as my anxiety was lil better.

I am getting breathless while speaking today and can't pronounce words which are tough as my tongue twists while pronouncing them, is it because of change of quetiapine from SR to IR? If so, will it settle down with time?

The slight breathlessness or twisting of tongue while talking are probably occurring due to anxiety. These are mild symptoms and ignoring these symptoms will make you better. Just try not to give attention to these symptoms. Avoid catching breath again and again. Don't take rapid shallow breathing. Shifting from SR to IR Quetiapine is not the cause of these symptoms. Just try to ignore these and this will settle with time.
Yes sleep will improve with time. SR tablets result in maintaining the sleep. IR acts rapidly but the sleep will improve with time.
Changing from SR to IR doesn't cause any serious effects except sedation.
Lurasidone will work in controlling the symptoms, just wait for few more days. The breathlessness will stop with time while on Lurasidone.

Quetiapine helps in sleep but some times higher doses are needed to get desired effects. If you are not taking Zapiz then increasing the dose of Quetiapine can help in sleep. This will improve the sleep.
If you are getting hiccups and uneasiness immediately after taking IR then that could be the reason. But that would cause only mild symptoms.
Lurasidone doesn't cause sleeplessness. It has not very strong effects on sleep. Wait for a week and see if improvement occurs. If symptoms persist then increasing the dose of Lurasidone is option.
For sleep if possible talk to your doctor and ask for increased doses of Quetiapine. Alternatively increasing the dose of Amitriptyline is another option. This will help to regularise the sleep.
IR helps in immediate onset of response while SR helps to maintain long duration of effect. IR or SR can both be taken, SR will help in refreshing sleep. Half life is about 7 hours while for SR its about 10-12 hours. This difference is not the reason for breathlessness you're having.

It can't be quantified that 50 ir is equal to 100 sr but as I have mentioned in last answer that in terms of acute onset of actions 50 IR is almost equal to 100 SR. So even on abrupt shift you will not face any withdrawal symptoms. It can be said that 50 IR is equal to 100 SR in acute actions.
Please continue taking Pantoprazole D empty stomach in morning and in evening you should take 20 mg Pantoprazole plain about 2 hours before dinner. Pantoprazole is longer acting and in normal cases only once a day doses are given, but as you are having stomach issues so evening dose would definitely do good. In your case morning and evening doses would do better.

Quetiapine, Sizopin, Amitriptyline and even Lurasid have anticholinergic effects which result in dry mouth, blurred vision, constipation etc side effects. The tremors could be occurring due to three antipsychotics you are taking.
Reducing SR 100 to 75 IR is not the reason for the symptoms you are having.
Reducing the dose of Quetiapine to 50 mg will not cause any withdrawal symptoms. But this should help in dry mouth, blurred vision, constipation etc symptoms.
Sizopine also results in good sleep. But the drug also causes dry mouth, blurred vision, constipation etc side effects. But I'd advise you to talk to your doctor and ask for complete dose reduction of Quetiapine and if possible Lurasidone. Increasing the dose of Sizopin and stopping Quetiapine and Lurasidone will help in better functioning. In my opinion giving three antipsychotics simultaneously in subtherapeuctic doses is not a wise thing compared to giving Clozapine in good doses. This will reduce the side effect risk and will help in better functioning.
Lurasidone is not the resin for disturbed sleep. You are on very low dose and I don't think Lurasidone is the reason. Sleeplessness in your case is probably due to persisting anxiety and may be due to Zapiz.
I prefer to advise it in evening but later 40 MG twice a day is given. You can take it in morning without worries.

During my last visit to the doctor on tuesday (24th Jan), quetiapine was reduced to 50 from 75, sizopin was increased to 25 from 12.5 and zapiz was reduced to 0.25 from 0.5. With this change, i have observing internal shakiness (though it was there earlier also, it started with changing quetiapine from sr 100 to ir 100 and continuing after making the change in the medication), will it go with the time.

Increasing the dose of Sizopin is a good move. In future request your doctor to stop the Quetipin completely and if needed increase the dose of Sizopin. You are already on three different antipsychotic drugs.
Glycolate will help to beat the shakiness, salivation etc symptoms. The drug is safe to use. Even it can also cause mild dryness of mouth but that would be tolerable.
