Does Metronidazole Gel Cause Irritation And Leave Marks On Skin?
Question: Hi there I have pd and before this month haven't had a flare up since September. I got one since the temp changed for me and I started using metro gel once a night. It was making my face more stressed and red so I stopped. I stopped two days ago and used my soap and cream products , and it got a little better. My question is if the metro gel made my face a little more red in the areas I put it on, will that leave a mark? I'm thinking the redness will go down, but wanted to check to see if thime metro gel can scare the face like that.

Metronidazole gel can cause some irritation, specially in sensitive skin of PD which can get easily irritated.
Usually this is not worrisome... just a mild redness, dryness and itching.
Though metronidazole is an effective remedy for treatment of PD. but irritation and dryness is a known side effect of topical metronidazole.
Some people may not tolerate it that well. You did the right thing by discontinuing it.
No, it won't leave a mark..

-Are you still taking Oracea? If not, you may have to restart it..

I am still taking Oracea, I started it in September and have not stopped since. My face is clear and just a little red from the metro gel. Honestly a picture will show nothing, I have my face back, just a few zit scars on my chin need clearing. I read that calendula cream does a very good job in clearing and soothing pd. I think if I have another flare up I will use thst instead of any topical creams. Those seem to always irritate my face and make things worse. Have you heard about the calendula cream? Do you like it?

Calendula is not an approved treatment for pd.
Nevertheless it said to have antiinflammatory properties and some people may find that there pd improves somewhat with topical calendula. Therefore you may try it.
I do get patients who are already using calendula beforehand but I find that it is not a very useful product for pd and ultimately my patients want to try something else.
Therefore I don't prefer it in my patients of pd or rosacea.
Oral treatment is the mainstay of treatment and some dermatologist prefer to not give any topical at all because pd patients have a sensitive skin and easily prone to irritation.
PD responds best to Oral antiinflammatory antibiotics, particularly tetracycline group of antibiotics like doxycycline, minocycline etc and macrolide group of antibiotics like erythromycin, azithromycin and roxithromycin.
They may need to be continued for 3-4 months and for a good time after the disease is under control
Nevertheless mild perioral dermatitis may respond to just topical treatments.
Topical treatment options include topical antiacne antibacterials like clindamycin, benzoyl peroxide, nadifloxacin for bumps/ zits and topical metronidazole gel.
Topical calcineurin inhibitors like pimecrolimus 1% cream is also an effective treatment option for pd and I would suggest you to try this next time you have a flare of pd. It is approved for treatment of pd and I find good results in my patients of pd.
