Enlargement of male breast(gynecomastia) requires further evaluation for identification of underlying cause.
Since your blood work up has already shown elevated prolactin, the cause for elevated prolactin should be identified.
Increased prolactin levels can occur due to certain medications, pituitary adenoma, hypothyroidism (a major concern in your case since you are complaining of low energy) ,liver/renal problems etc. I recommend that you need to undergo Complete metabolic profile, MRI brain(especially if prolactin level was more than 5000), thyroid function test at this point. Correction of underlying cause can reverse the gynecomastia.Also,please let me know the list of any medications you are currently taking to identify possible offending medication.
If a particular underlying cause cannot be identified, Then Tamoxifen, anastrazole can be used for management of gynecomastia. Otherwise correction of underlying cause is the best option.Increased prolactin levels is best treated with medications like bromocriptine and cabergoline.
If you notice any lumps in the breast, Then a biopsy is also indicated.
There are no over the counter medications available for gynecomastia.

Hello again,
I have gone through your medication list. Of all the medications you are taking only hydrocodone and trazadone have a minimal risk of causing gynecomastia.There are case reports showing increased prolactin level and gynecomastia in patients taking trazadone.if possible,you may try discontinuing these medications. However, the risk is low and discontinuation of hydrocodone/trazadone is not a must.
But since you have been diagnosed with Alpha one antitrypsin deficiency, I recommend getting liver function test done.Decreased liver function can result in development of male breast
Liver function test will be done as a part of the complete metabolic profile I had mentioned in above answer.
