The thing with antibiotics is that they are given for particular duration of time in specific indicated doses.
Your doctor would decide to give which antibiotic for that particular infection to cover the organisms involved in how much quantity and for how much period of time.
Usually when an antibiotic dont work we replace it with the stronger one.
So you should complete the duration and dose prescribed by your doctor
If you are fasting and you are not able to take the the required amount of drug that will effect the outcome you should talk to your doctor as either to adjust the doses so that is feasible and won't effect your fast like from three times a day to two times a day or if that is not possible you have to wait for the completion of the course of antibiotics.
Regarding your skin problem it seems that your skin may be sensitive so a minute insect bite and cut can prove very difficult to heal and more likely to be infected so it is possible that if the skin is taking time to heal or there is delayed healing secondary infection is prevented by antibiotics.
If your skin condition is not getting any better with the antibiotics then it require futher evaluation.
