However, in those who continue to have distressing hot flushes after menopause, use of oral hormonal pills are recommended for a brief period.
Estrogen vaginal cream is a low dose estrogen preperation and is used in atropic vaginitis ( dryness) in post menopause age and in ladies with recurrent urinary tract infection. Intermittent ( weekly twice or thrice) use of low dose vaginal cream is good and safe.
Progesterone is generally combined with estrogen in pills to avoid endometrial proliferation in ladies who have uterus intact. In ladies with out uterus ( those who had surgery), estrogen alone pills are used.

Good day.
Endometrium is the inner lining of uterus. Before menopause, normal estrogen levels in a lady cause endometrium to grow ( endometrial proliferation). This sheds during menstural periods. In the absence of progesterone, mesntural periods may not happen regularly and this cause the endometrium to become thicker and thicker. This can give trouble later in life. However, this doesnt happen if the lady has regular menstural periods because of endometrial shedding with periods.
. After menopause, endometrium shrinks as well as your uterus and ovaries.
When you use estrogen alone ( in pills), the endometrium start to grow again. If you do not add progesterone, endometrium do not shed and it becomes thicker.
That is the logic behind giving a combined ( estrogen+progesterone) pill in a lady with intact uterus. However, when we use estrogen as a vaginal cream, the dose used in extremly low and this may not cause this endometrial growth.

Estriol repacement is considered to be safer. It is another type of estrogen derivativ.
however, you must check with the local authorities regardng the recommendations in your counry regarding estriol use.
To be XXXXXXX my experience with Estriol is very limited for the simple reason that it is not widely used here. However, many reports says that it is safer than estrogen pills.
