Brief Answer:
I may suggest following-
Detailed Answer:
In such a case I may suggest to follow second regimen (Istamet (50+1000), Volix 0.3, Glumisave)
But Glimisave in slightly lower dose.
Tab Glimisave contains Glimepiride which act on pancreas to release insulin.
Considering your age and current medicines there may not be adequate pancreatic reserve to respond to this type of medicines. This medicine may fail as duration of diabetes advances. Also has more risk of developing low blood sugar level (hypoglycemia) with slightest alteration in diet, physical activity or even with minor illness.
In my practice I rarely suggest more than 4mg of glimepiride per day.
Along with this follow diabetic diet and exercise as tolerated.
Repeat fasting and post-prandial blood sugar after 3 to 4 weeks depending upon it further changes in medicines can be done.
Keep regular follow up with your treating diabetologist.
Hope this helps you.
