Does Intake Of Omez And Nasonex Help In Treating Pharyngitis?

default
Posted on Fri, 1 Jul 2016
Question: one ENT Dr i was seeing, observed mild pharyngitis gave me odemot LC for 2 weeks, Omez for 3 weeks and Nasonex spray (bed time) for 10 weeks. I saw her last past 10 days, she also felt lot of improvement and i also fell much better than earlier. However, slight thorat irritation is still there. Which gets reduced when jammed mucus from throat is released either automoatically or after salt water gargle. Please guide me if the life of treatment is correct. Additional medicine i am taking through a Physician is Telma AM(OD) for BP and foracort rota caps (OD).

Brief Answer:
Take tablet Mucinex-Dm instead of Odemont-LC.

Detailed Answer:
Hello,
Welcome to HCM,

I have read your query and reviewed your doctor's prescription attached here.

From the facts you have shared here,it seems that you are having cough with mucus instead of having dry or non-productive cough.

While Odemont-LC contains Leukotriene receptor antagonists (montelukast) with antihistamine (levocetrizine) which is a cough suppressant combination. It will help to reduce symptom like sinus congestion, runny nose, sneezing, and dry cough but NOT wet cough (means coughing with mucus or phlegm).

So,in my opinion you should stop taking Odemont-LC further, instead of it you should take tablet Mucinex-Dm (available OTC) twice daily which contains Dextromethorphan, a cough suppressant in combination with Guaifenesin, an expectorant and helps to loosen congestion in your throat and making it easier to cough out mucus/phlegm.

You can continue with Nasonex spray and Omez with other medications like Telma AM for BP and foracort rota caps.

Continue with salt water gargles with steam inhalation 3 to 4 times daily which will also be helpful to remove cough from throat.

Take plenty of fluids with warm water or tea and better to avoid cold water/drinks.

Dr Please note that i am not taking Odemont since last 6 weeks, At present Only medicine which i was taking for pharyngitis was Nasonex. Which is also stopped by Dr since last 4 days for next 2 weeks, to understand and diagnose the problem better.
Further, please note that at present i am not having any cough. But i had in the sever cough and cold in the month of December for more than 3 weeks. Same got reliefed after foracort etc.
SOmetimes i feel right ear getting blocked from inside , which get reliefed after i move my jaw like chewing also relief is observed when i expell the jammed mucus forcefully.
Now please further suggest any medicine for gargle to get relief from throat irritation and ear jam feeling.

Brief Answer:
continue with salt water gargles.

Detailed Answer:
Hi,
Thanks for sharing more information,

Usually cough with mucus, runny nose with other sinus symptoms are viral in origin and usually do not require any steroids or antibiotics or any other specific medications. They get relieved with symptomatic medications only.

For pharyngitis, you should continue with salt water gargles which would be the best since it is unlikely to be having some bacterial infection,so there is no need to take antiseptic mouth wash or gargles.

However, you can take over the counter antihistamine like levocetrizine 10 mg for throat irritation as you are no longer having cough with mucus.

For ear jam feeling,it is likely because of excess wax deposition (also written in your doctor's prescription) I would suggest to use Soliwax or Waxsol Eye drops after asking to your treating doctor.

Let me know if you need more help.

Please answer the following with recommendations:
1. How would the eye drop help in ear wax?
2. Further, if it was type error and you mean to say ear drops, how the release of mucus is easing the ear jam feeling if it is due to wax?
3. Is the line of treatment good?
4. Refer the prescription i uploaded earlier and advice, is there any possibility that i need to go for nasal surgery? or it shall be all well without surgery.
5. Shall i take all the medicines advised by you in both first and second response and for how many days?
Any other advice you feel necessary to be provided, please recommen.

Brief Answer:
No need to take all the medicines.

Detailed Answer:
Hello,
Thanks for the response,

Yes,it is actually a EAR drop instead of eye drops which I wrote by mistake and ear wax could be the contributory factor for the feeling ear jam.

The relief of ear jam after the release of mucus could be commented only after complete ear and throat examination by otoscope and pharyngoscope respectively.

But you can use war remover ear drops (soliwax) as I mentioned.

Regarding nasal surgery (for DNS+ as written in your doctor's prescription),it depends on the degree of septal deviation (moderate or severe) and how frequently you get nasal or sinus infections. Your ENT doctor will decide when to go for DNS surgery because it is difficult to say without doing Nasal examination for DNS.

Since you have no longer cough with mucus as you said earlier,I would suggest only tablet levocetrizine 10mg for your throat irritation until it subsides.

And continue with salt water gargles with nasal spray, there is no need to take any additional medicines.

Dr.Sandip Kabra
