Lymphedema during therapy of tumors is caused due to radiation induced damage of lymphatics or surgical removal of lymphnodes.( Damage to or blockage of lymphatic system)
In all cases reportedly associated with myelodysplastic syndromes, primary lymphedema is present (pathological fault or congenital abnormality in the lymphatics) and lymphedema starts at an early age (before middle age)
Germline GATA -2 mutataion is commonly associated with this abnormality and this condition is called as Emberger syndrome. GATA -2 haploinsufficiency mutation is known to affect gene regulation during vascular development and blood cell differentiation.
If it is secondary after only chemotherapy, few drugs needs to be ruled out as cause of lymphedema.

Yes, few cases of lymphedema have been noted with myelodysplastic syndromes and they are cases of Emberger syndrome.
Other causes of lymphedema we can think of are due to obstruction secondary to infection by parasites or any tumor infiltrating or compressing the lymphnodes.
