Does Prostitis Elevate PSA Level?

Question: I was diagnosed with uti infection (klebsiella pneumoniae) now they think I have prostitis..I was on antibiotics for 6 weeks...4 weeks after the infection..I started having discomfort around the anus when sitting.
My doctor referred me to urologist. ..he did a cystoscopy ..bladder and Prostate looked normal, when he performed DRE He felt that the right side was abnormal..he ordered me to to get PSA test done..he told the level might be high because of the infection, infection or antibiotics.
My PSA was 5..he said it's not that high and performed another DRE test ...he said he does not think it's something bad..my abnormality could be due an abscess..he said he does not feel that my prostate has any signs of cancer abnormality. Now I'm having an ultrasound done after tomorrow.
blood test in 5 weeks..and i will see the urologist on the 22nd of October
My question is can prostitis cause this ?? Does prostitis elevate the PSA level?? Do you think I should Take a longer course of antibiotics? ? What do you about prostate abscess? ?

Thanks for your query,based on the facts that you have posted it appears that you have been treated with antibiotics for Prostatitis.You have already finished a course of Ciprofloxacin for 4 weeks and you recent serum PSA is reported to be 5

I shall be happy if you can post me few more details to help me in making comments on your problem.
1) Was Serum PSA done before starting Ciprofloxacin ? if yes what was the report?
2) Was Trans Rectal Ultrasound Scanning (TRUS) done at any time before diagnosis of Prostatitis?
3)Was swab test and culture of the urethral discharge collected after DRE done to confirm the diagnosis of Prostatitis?

Since you are feeling better after taking antibiotics for 4 weeks it suggests that infection is getting cleared off .As such prostatitis takes long time to get cured completely and sometimes patient has to take antibiotics and anti inflammatory medications for 3-4 months .

I would like to answer your queries point to point.
1) Can prostatitis cause this ?
Ans_ Yes prostatitis cause these symptoms like perineal pain ,pain in penis ,pain around anus etc
2) Does Prostatitis elevate PSA level?
Ans -Yes Prostatitis does raise PSA level .
3) Shall I take antibiotics for longer duration?
Ans -Yes sometimes one has to take antibiotics for 3-4 months for complete cure.
4) About Prostate abscess?
Ans -Prostatic abscess can be diagnosed on DRE and is confirmed only on Trans Rectal Ultrasound scanning .

Please repeat your Serum PSA at regular intervals of every month.In case If there is persistent raising trend in PSA levels one has to think of doing Ultrasound guided multi core Prostate biopsy to rule out Cancer of prostate.

I would rather suggest you to do culture and sensitivity of urethral discharge to find out the organisms causing this infection and antibiotics to which they are sensitive to so that you can take appropriate antibiotics as per culture report.to combat infection earlier.

More and accurate information:
1-I'm still on antibiotics, today is going to be my

2- my PSA was done while I was still on antibiotics, and 15 mins after having a cystoscopy done (he told me

To be honest I don't know why he made me do the PSA knowing that it might be high because of the infection and because of the cystoscopy. He should have waited at least 3 day so my PSA can go down to normal level.

3-i did not have any TRUS done to confirm the diagnosis of prostitis. .i was diagnosed with Uti infection and then i saw different doctors in ER..
everytime i had different opinions based on DRE

the TRUS I'm having it tomorrow for the first time.
now i have discomfort ans sometimes a bit of mild pain in my anus area..the discomfort started a month ago and that's the reason why i think I'm not healing completely.
The urologist wrote on TRUS request this" unsual
case pelvic pain..very abnormal feel right lobe of prostate..prostate abscess? ?"

I don't know why he wrote pelvic pain because I only feel discomfort...and i don't why an ultrasound was not at very beginning.
Now if the TRUS confirm an abscess? ? Should I wait or should I go to ER..I live in Canada and my next appointment with my urologist is until next October 22nd

Hi,
I had TRUS done today..and was told that there is no
Abscess in the prostate ( I didn't get a copy of the report
It takes 4 days he said)
now that I ended my course of antibiotics and l still feel
Discomfort when sitting ..what should I do??

Thanks for writing again .Continue taking antibiotics for 2 more weeks .If discomfort persists I would advise you to get prostatic massage done every day.This is the practice I am following for my patients with chronic Prostatitis since last 40 years and gives encouraging results.
prostate massage reduces prostate congestion and there by helps to relieve pelvic pain and discomfort..You can try this .

Hi Doctor,
i saw my family doctor yesterday and he wants me to do PSA test knowing that i just had TRUS done 3 days ago... i told him that because of TRUS i should wait longer beofre having PSA test but he said its ok to do it.

what if my prostatitis is not cured yet ?? will affect my PSA levels?

the ultrasound report reads as follow:
the bladder walls are normal but the bladder is poorly distended with a volume only of 85 cc, post void residual is 10 cc indicating a retention up 5 %.

the prostate is mildly enlarged with a volume of 30 cc, it is grossly homogeneous with no nodularities but it has some calcification
There are no pelvic masses
SUMMARY
Mild prostatic hypertrophy with a small retention in the bladder after voiding.

Your ultrasound report suggests mild enlargement of prostate with post void residual urine of 10 ml a ,calcification in prostate is suggestive of Chronic infection of prostate.

Continue taking antibiotics for 2 more weeks along with Tomsulosin once a day
