The plan would be:
Semen analysis is the first and the best investigations. This should be done after abstinence (No ejaculation) of 5 days. To be done at 2 - 3 different labs to get a confirmed diagnosis.
Clinical evaluation, examination by a General Surgeon, preferably the Urologist to correlated the reports and findings.
Ultrasonography and color doppler of the scrotum.
The will give the primary diagnosis to go ahead with further tests and plan the treatment accordingly.

Please repeat the test as advised after a gap of 5 days of no sex/ejaculation form some other lab to confirm the reports.
Urologist's opinion and Ultrasonography as advised.
Then consult a Fertility center and see if Intracytoplasmic Sperm Injection - ICSI and IVF can be done as this is highly effective treatment for male factor infertility problems.
