Does Testicular Pain Indicate Testicular Torsion Or Epididymitis?

Question: Hey doctor,

Since Tueaday I have been having this dull ache in my right testicle that radiates to the right side of my thigh/groin and my very lower right abdomen but its hard to pinpoint exactly where the pain is. It hurts a little more if I press my right thigh as well. I went to the doctor and had a urine test which I am waiting for the results on and I actually had a lower abdomen ultrasound the week before because of these bowel problems I have been having which came up clear. What could it possibly be I have read up a few things testicular torsion which I assume it isn't because apparently it is very painful?and epididymitis which it might be but how exactly is that diagnosed usually would it be through a urine test? What can I try to allivate the pain as well? I tried two Pandols but it didn't really do anything is there anything else I can try? Bear in mind I do have acid reflux problems as well.

Hello!

Thank you for the query.

Testicle torsion is an urgent condition which leads to testicle necrosis. So this is not it for sure.
You may have some kind of inflammation including epididymitis or testicle inflammation. If so, there might be no urine changes, just the pain. Varicocele as well as a hernia should be considered as well. Those might not be visible in regular ultrasound. Finally prostate inflammation can give such symptoms as well.
It would be good to consult urologist. Testis ultrasound and spermatic cord ultrasound are the first tests to be done. Rectal prostate examination seems necessary as well.
Depending of the reason, antibiotic might be the best thing for the pain.

Thanks for that doctor i'll definitely head back to the doctors and try and get those tests done. One other thing I have noticed which I have attached an image of is a little red pimple type of thing on my balls is this normal alog with red looking veins? I won't be able to get it seen to until Monday at the earliest since it is the weekend would this be a bad thing and would it hurt me waiting that long is it worth going to the hostipal to get checked out or do you think it could wait? The pain isn't severe and I am meant to work tomorrow standing all day but it is mild and Pandol didn't really do anything is there anything else I can try until I get it seen?

The pimple thing is just a separate issue. It is a hair follicle inflammation. Nothing really serious. So it can wait. It can even resolve by its own.
Veins are very normal thing and you can ignore them.
You may try Diclophenac instead of Pandol for the pain management.

Hey doctor just an update the pain has kind of gone away from my testicles and more so in my right abodmen and also in my left abomen I have attached pictures of where exactly . I have also started to get a diarrhea and before it comes I get like a painful gnawing type of pain throughout my entire lower abdomen that makes me need to go badly. But when its calmed dowm I kind of just have a constant ache that switches between my left and right sides and it seems to be worse on my left side after I eat, but I have kind of loss my appeitate and haven't eaten a huge amount. I went to the doctor and asked if it was appendicitis possibly but he didn't think it was as my pain isn't severe enough what else could it be and what should I do?

Bladder infection. It is not appendicitis.

Lower abdomen is where a bladder is located. So bladder infection is the most probable reason. If you have cramping pain which comes and goes, intestinal issue including bowels obstruction should be considered.
In my opinion you should have urine analysis and urine swab test done and at least abdominal x-ray to rule out obstruction.

I've already had two urine tests which came out completely clear and an ultrasound of my lower abodmen but not my bladder about a week ago which was clear which is why I am a bit hestiant. The doctor prescribed me antibitoics Augmentin Duo Forte 875mg, but I am not sure if I want to start taking them as I am currently have a lot go gastro problems with diareha and stomach cramps on top of the lower pain i described should I maybe wait to those subsised before trying the antibitoics as I know they can cause a lot of stomach issues?

Also to add I am not really sexually active either and the doctor thinks epididymis which I am not sure that it is that.

Well. Antibiotics are for infection. But first it would be good to diagnose it. Taking tham just like that is not a good idea.
Urine swab test is something you need. And urologist consult. Without it you can not move forward. You should also have your stool checked.

I just came back today with a blood test with a 7.6 reading of blood sugar level and I have had this lain where my pancreas is the last month or so. What are the chances of pancreas cancer in a 19 year old and what tests can I get done for it? I had an ultrasound last week which came up clear but I am unsure how accurate they are? I have attached images of my results.

Hi, I've reviewed them,

The chance for this cancer at your age is extremely low.

The best test for pancreas diagnostic is CT-scan. Ultrasound is not a good test as it can not reach pancreas in most cases.
