Posted on Sat, 18 Oct 2014
Question: My mom was suffering from sever abdomen pain & doc suggested CT of abdomen..Ct reveals left lobe of liver is enlarged with altered parenchymal texture & prominence of interlobar fissure suggestive of liver cirrhosis.
In LFT GGTP reading is 60U/l while others are normal, PT is also normalHCV is negative & ANA is also negative.
However alpha feto protein value is 5.17.. Is this suggestive of cancer??

Brief Answer:
Not suggestive of cancer

Detailed Answer:
Hi

As per the CT scan of abdomen report there is no space occupying lesion in the liver and the increased alphafetoprotein (AFP) and Gammaglutamyl transpeptidase (GGTP) of your mother is due to cirrhosis of liver and not due to cancer.

Liver cancer generally presents with upper abdominal pain, abdominal swelling ,white chalky stools, loss of appetite and yellow discoloration of skin and the whites of eyes.

Since her bilirubin, SGOT and SGPT report in blood are normal and HCV and ANA also is negative she does not need to worry and is not suggestive of cancer.

I will advise your mother to eat a low sodium diet.

She needs to eat a healthy diet that includes a variety of fruits and vegetables.

She must choose lean protein, such as legumes, poultry or fish and avoid raw seafood.

Brief Answer:
MRI of the liver

Detailed Answer:
Hi

Upper abdominal pain and increased SGOT and SGPT can also be due to cirrhosis of liver.

An AFP greater than 200 with a liver lesion greater than 2 cm is highly suggestive of liver cancer.

Your moms AFP is less than 200 and there is no lesion in the CT scan abdomen and it is highly unlikely to be liver cancer.

I will advise her to do a triphasic dynamic contrast enhanced magnetic resonance imaging (MRI) of the liver.

A lesion greater than 2 cm demonstrating classic arterial enhancement on MRI is diagnostic of liver cancer.

Dicuss the above findings with a radiologist after you have done the MRI to rule out liver cancer.

If you have no more clarifications then please rate the answer and close the thread.
