Feeling Discomfort And Pain In Chest, Throbbing Sensation In Arms. Diagnosed With Acid Reflux. Concerned

Question: HI

Since last few weeks I am having discomfort in my whole chest and pain at a particular area in the chest my left hand and shoulder also have a uncomfortable feeling throughout the day.
I am also feeling slight out of breadth when I am talking loudly (at normal pace everything is fine).I am also having dry cough from about 2 months.

Also I am having throbbing (Fasdiculations) in the arms and chest area frequently.

I was diagnosed with acid reflux and Cervical Spondlysis about 2 months back.
I am sure this is not a heart problem but since the discomfort is on the left side which is bothering me.
I had a ECG and a 2D doppler done in the month of April when I was diagnosed with acid reflux the results were:

ECG(Borderline ECG)- 1-Sinus Rhythm 2-T wave Inversion at lead III
2D Doppler Cardiography- Trivial Tricuspid Regurgitation and Mild Pulmonary regurgitation PAEDP=7.26mmhg other things are normal.

The doctor that time said everything was normal with the heart..
I am taking a medicine called NEOPRIDE -TOTAL every morning empty stomach.
please suggest whaty needs to be done because its quite irritating with the whole arm and shoulder discomfort and Fasdiculations. and my head is also giving me a weird feeling (no pain) also my legs also have a uncomfortable feeling.

Thanks for your query.

It seems as if you have been suffering from Gastro esophageal reflux disease (GERD) or peptic ulcer disease for a long time.

GERD is a condition where the food from the stomach enters the food pipe to cause pain in the chest which may sometimes radiate to the shoulder but it’s better to do a X-ray shoulder joint to see any shoulder pathology.

You have mild cough that may be due to GERD itself but it can be due to aspiration pneumonitis. So do a Chest X XXXXXXX to rule out any lung involvement.

Your ECG & 2D-ECHO are within normal limit according to your doctor (as I din't get your ECG report) so do not be worried about heart disease.

The following lifestyle modifications should help.
1. Avoid oily, spicy, fatty food, alcohol and smoking.
2. Take small amount of food frequently.
3. Avoid lying down within 1.5 hours after meal.
4. Continue NEOPRIDE –TOTAL for 6 weeks.

Consult your physician, test to de done and follow his advice for further management.

what about the Fasdiculations and cervical spondilysis and weird feeling in my head ..are they due to GERD or Cervical Spondilysis.

Thanks for your query.
Yes the headaches and throbbing in the arm is most likely to be due to your cervical spondylosis.
Pain of cervical spondylosis can radiate to the head and shoulder.
Based on the severity on X-ray picture you need
1.     Short course of Pain killers (NSAID) with muscle relaxant to reduce the pain and stiffness.
2.     Physiotherapy like Ultrasonic therapy, intermittent cervical cord traction etc. It should be applied by a qualified physiotherapist to reduce unwanted side effects.
3.     Regular neck exercise.
4.     May need cervical collar.

Please consult an orthopedic surgeon for final opinion regarding management plan.

can you please let me know which are the tests to be conducted so that i can complete them before making a visit to the doctor..
also I am having weakness in my arms and legs and strange headache at the back of the head is it due to cervical spondylysis.

Thanks for your query.
Headache, weakness and pain in shoulder joint are probably due to cervical spondylosis but weakness in the arm anyhow not related with cervical spondylosis.
You need to do X-ray cervical cord (AP & Lateral view), X-ray both shoulder joint (AP & Lateral view), Blood for fasting sugar and uric acid estimation.
Consult an orthopedic surgeon, test to be done and maintain his advice about the management.
