First of all, please note that as so many doctors have seen your husband and have been able to examine him and perform some tests, it will be very hard to solve your problem from my point. But lets try.
It would be much easier having the results of previously performed tests. Especially full blood work, urine analysis, urine culture tests, ultrasound and X-rays.
Generally groin area pain can be caused by urinary tract stones (it appears suddenly and is very strong sometimes unbearable, can be a reason of collapsing), urinary tract infection (during the hernia repair, there is always a risk of urinary bladder injury, if not diagnosed can be a reason of severe hard treatable urine infection), testicle inflammation, testicle torsion, another hernia.
It is also possible that during the previous hernia repair, the nerve has been stitched and gives such symptoms.

Well, this hole can be an XXXXXXX inguinal ring which is the path for hernia. Please perform simple test. Ask you husband to stand up (if possible), put your finger into this hole and ask him to cough. If there is a hernia out there, you should be able to feel movable lump.
Urine severe infection is still possible. There might be an antibiotics resistant bacteria which can give symptoms when immune system gets weak or the urine does not flush urinary tract often enough.
The best way to diagnose the hernia is to perform ultrasound of painful groin (you havent mentioned if there was any performed), testicles ultrasound. Urine analysis and urine culture should be also done.
